The Role

Homeward is rearchitecting the delivery of health and care in partnership with communities everywhere, starting in rural America. Today, 60 million Americans living in rural communities are facing a crisis of access to care. In the U.S. healthcare system, rural Americans experience significantly poorer clinical outcomes. This trend is rapidly accelerating as rural hospitals close and physician shortages increase, exacerbating health disparities. In fact, Americans living in rural communities suffer a mortality rate 23 percent higher than those in urban communities, in part because of the lack of access to quality care.

About the Role

We’re looking for a high-agency Product Manager who loves to build, experiment, and ship. You’ll lead the development of AI-native tools that power how we deliver care to complex, underserved populations. This is a deeply cross-functional role where you’ll work closely with clinical, engineering, and data teams to turn messy real-world workflows into scalable, intelligent systems.

If you’re the type who prototypes ideas on your own, jumps into new tools for fun, and thrives in ambiguity, you’ll fit right in.

What You’ll Do

  • Own the delivery of our AI product roadmap, from discovery to quick experiments to scalable solutions.

  • Partner with clinicians, engineers, and data science to build AI-powered decision support tools and care coordination products.

  • Design and ship systems that help care teams work smarter: automate repetitive tasks, surface insights, and improve care quality.

  • Run rapid learning cycles that balance clinical rigor, operational reality, and great user experience.

  • Build foundational infrastructure, workflows, and tooling that enable care teams to scale sustainably.

  • Prototype and test ideas hands-on, from simple scripts to lightweight AI demos, to accelerate learning.

What You Bring

  • 3–5 years of product or builder experience in a technical, operational, or early-stage environment.

  • A track record of building things; side projects, prototypes, hackathon projects, automations, internal tools, anything that shows bias toward action.

  • Strong technical aptitude and curiosity; you enjoy exploring new AI tools and understanding how things work under the hood (no need to train models, but you should know how to ask sharp questions).

  • Comfortable operating in messy, real-world workflows with clinicians, operations, and engineers.

  • Strong product intuition, user empathy, and an obsession with learning quickly from real usage.

  • High ownership, humility, and coachability.

Nice to Have

  • Experience in healthcare, clinical operations, or care coordination.

  • Exposure to value-based care, home-based care models, or complex care delivery.

  • Hands-on work with data/ML teams or personal tinkering with AI systems.

  • Background in engineering or a “jack-of-all-trades” builder role at an early-stage startup.

The base salary range for this position is $140k-$170k annually. Compensation may vary outside of this range depending on a number of factors, including a candidate’s qualifications, skills, location, competencies and experience. Base pay is one part of the Total Package that is provided to compensate and recognize employees for their work at Homeward Health. This role is eligible for an annual bonus, stock options, as well as a comprehensive benefits package.
