Healix Infusion Therapy is seeking a PRN Clinical Pharmacist for one of our Infectious Disease Office Infusion Centers located in San Antonio, TX!


Our Clinical Staff Pharmacist oversee onsite infusion pharmacies and are responsible for the compounding, dispensing, and monitoring of medications for patient use at home or in the infusion center. The Pharmacist conduct clinical consultations frequently with physicians regarding drug therapy and interact daily with the office infusion center nursing staff to coordinate patients’ medication therapy plans.


SCHEDULE: Standard day time clinic operating hours. Looking for someone with weekday availability to pick up shifts.


ESSENTIAL DUTIES AND RESPONSIBILITIES:

Ensures that drugs and/or devices are dispensed and delivered safely and accurately. This is accomplished by fulfilling and dispensing orders appropriately, identifying, resolving and preventing potential and actual drug-related problems, and optimizing patient therapy outcomes. Directs and coordinates pharmacy and site operations as requested.

  • Ensures that the pharmacy meets all state and federal laws and regulations, while working in the pharmacy.
  • Promptly and accurately receives, interprets, evaluates, prepares, and dispenses prescription drug orders.
  • Monitors therapy for each patient, perform pharmacokinetic monitoring and maintains appropriate drug use for each patient and client. This is evidenced by audit scores on pharmacy assessments, care plans and progress notes, medication profile reviews and updates, and therapeutic outcomes.
  • Provides to the patient or patient’s agent accurate and meaningful verbal and written drug and/or device information.
  • Monitors therapy for each patient, performs pharmacokinetic monitoring and maintains appropriate drug use for each patient and client.
  • Responsible to maintain required production and dispensing duties accurately as the assigned shift requires.
  • Maintains appropriate oversight of the intravenous compounding and checks all pharmacist-required compounds prior to mixing.
  • Insures that the clean room is maintained according to policy and law and regulation.
  • Complies with Healix Policies and procedures and recommends modifications as necessary.
  • Arrives at site promptly and is present for all assigned shifts. Notifies manager and Pharmacy Staffing as soon as possible for any anticipated outages.
  • Selects cost-effective and clinically appropriate drug products, methods of delivery, and delivery systems as permitted by prescribing physician.
  • Develops, implements, and/or enhances pharmacy care programs and/or services that enhance and add value to the client’s practice.
  • Maintains optimal drug and supply inventory and reports are submitted in a timely basis.
  • Insures that contracted drugs are ordered, received, stored, and secured properly.
  • Insures that adequate security to prevent indiscriminate or unauthorized access to pharmacy and confidential patient records are maintained.
  • Communicates effectively with client.
  • Participates in pharmacy staff meetings on a regular basis.
  • Demonstrates a “team player” attitude with co-workers (on-site and corporate).
  • Maintains on-site documentation of current licensure, validation, training, as required for compounding pharmacists by State Board of Pharmacy.
  • Maintains clean and neat pharmacy.
  • Insures that patient charts are maintained, current, and complete according to state rules and regulations, accreditation agency standards, and Healix policies and procedures.
  • Responds to pages while on-call within 15 minutes.
  • Maintains ongoing quality improvement within the site.
  • Coordinates total patient care needs with the physician and nurse.

Healix® manages physician-owned office infusion centers and pharmacies nationwide providing in-office treatment and take-home medications for optimal patient comfort and outcomes. Our on-site clinical pharmacy team consists of Clinical Pharmacy Managers and Certified Pharmacy Technicians who provide an array of pharmaceutical care services to patients within Healix-managed office infusion centers across the country. Our pharmacists and technicians are able to enjoy daytime hours within a physician office setting and have direct patient interaction in an interdisciplinary team environment. This opportunity is located in San Antonio, TX.

Qualifications

Qualifications:

  • State Board of Pharmacy License, active and clear
  • Minimum 2 years of Clinical Pharmacy Experience
  • IV Infusion Experience preferred
  • Infectious Disease background preferred
  • Excellent Computer Skills

Clinical Skills Required:

  • Clinical and Pharmacokinetic Dose Monitoring
  • Sterile Products Preparation and Dispensing
  • Ability to provide physicians with clinical recommendations and dose adjustments
  • Patient and or caregiver counseling regarding therapy plans
