What You’ll Do

• Deliver personalized, data-driven care that combines leading-edge diagnostics with compassion and insight.
• Serve as the architect of each member’s health plan, translating results into clear, actionable strategies that drive tangible outcomes.
• Lead a high-performing care team, aligning Nurse Practitioners, Health Coaches, Nutritionists, Medical Assistants, and Care Coordinators around each member’s journey.
• Own the outcomes for your panel of members, ensuring clinical excellence, timely follow-through, and a seamless experience at every stage.
• Create trust through transparency, empowerment, and expert guidance, making complex science feel personal and achievable.
• Continuously refine care plans based on diagnostics, behavior, and progress, ensuring momentum, clarity, and precision throughout the year.
• Represent the highest standard of modern medicine, grounded in science, delivered with heart, and elevated by innovation.
• Support your team with the tools, structure, and inspiration they need to deliver care at a luxury, five-star standard.

Key Responsibilities

• Conduct comprehensive medical assessments that integrate history, lifestyle, labs, imaging, and diagnostic data.
• Create and present a fully personalized, long-term health strategy during post-diagnostic sessions.
• Monitor progress through follow-ups, adjusting care plans based on outcomes, updated testing, and member feedback.
• Lead ongoing care touchpoints that inspire adherence, clarify direction, and deepen engagement.
• Ensure complete care team alignment through proactive communication, documentation, and strategic delegation.
• Model precision, empathy, and professionalism in every interaction with both members and team members
• Provide clinical oversight for orders, prescriptions, imaging, referrals, and diagnostics throughout the year.
• Uphold Fountain Life standards of service, documentation, and clinical excellence.
• Participate in team huddles, case reviews, and member flow planning to ensure seamless experiences and strong outcomes.
• Prepare members for renewal by conducting final-year health reflections and implementing proactive planning for the next stage of their journey.

What You Bring

• Medical Doctor (MD) degree from an accredited medical school.
• Board certification or board eligibility in a relevant specialty (e.g., Internal Medicine, Family Medicine).
• Eligible to practice medicine in all 50 states.
• Proven clinical experience with strong diagnostic and treatment skills.
• Previous experience in longevity, preventative, or precision healthcare is highly preferred.
• Strong understanding of advanced medical technologies, preventative care strategies, and performance medicine.
• Excellent communication, organizational, and leadership abilities.
• Commitment to patient-centered, proactive health optimization.
• Strong ethical standards and adherence to HIPAA and regulatory guidelines.
• Ability to excel in a fast-paced, innovative environment.
