Summary/Objective

The Physician is expected to be familiar with the philosophy, goals and objectives of the PCMH model, and work collaboratively with other members of the patient care team to achieve maximal health outcomes for NSHC patients. In this role, the Physician will provide the full scope of primary care services for patients, which fall under his/her field of licensure and training. These services include: diagnosis, treatment, and coordination of care, for preventive and health maintenance to patients of all ages. The Physician must meet and maintain credentialing requirements for NSHC credentialing guidelines, as well as the credentialing guidelines for affiliated hospitals in Lake and Porter County, Indiana, as applicable, and must demonstrate clinical competency in the area of Family Medicine.
 

Essential Functions

  • Access health status of patient through health history, taking physical examination and diagnostic tests.
  • Review diagnostic reports (e.g. lab, x-ray, EKG) and access appropriate resources i.e. social services and behavioral health staff, etc.
  • Ensure the primary care team nursing staff follows up with the patient on diagnostic results, patient follow ups, etc. in a timely manner.
  • Develop and implement plan of care incorporating patient’s input in the development of the treatment plan.
  • Ensure direct counseling/education for each patient on a plan for treatment.
  • Prescribe medication in accordance with Indiana statute and professional practice

                guidelines

  • Perform minor surgical procedures.
  • Arrange referral for patients requiring services not offered at NorthShore Health Centers
  • Seek to promote maximal health among clinic patients by working in collaboration with the Patient Care Team to ensure each patient is treated as a whole, and has a satisfactory experience with their NSHC Primary Care Medical Home.
  • Responds to tasks and completes charting in a timely manner to prevent any delay in care and promote coordination of care. 
  • Participate in peer review, quality assurance review (including systematic review of records and treatment plans), provider meetings, team huddles, and other clinical meetings.
  • Participate in staff outreach and off-site health care programs in the community, as assigned by the Director of Family Medicine or the Chief Medical Officer.
  • Comply with the NorthShore Health Centers Infection Control plan which includes personally utilizing universal precautions.
  • Assist in updating protocols and principles of practice as requested.
  • Adhere to UDS clinical protocols to ensure requirements are met.
  • Oversee and collaborate with NorthShore’s Family Practice Nurse Practitioners in the provision of care for NorthShore Patients.
  • Prepares reports and other documentation as required.
  • Attends meetings and training sessions as required.
  • Executes other verbal or written specific assigned tasks, requiring similar or lesser

skills and abilities, some of which are continuing while others are occasional in nature.

Work Environment

Work is performed in an office environment. Involves frequent personal and telephone contact with patients, physicians, and other healthcare personnel. Work may be stressful at times. Interaction with others is constant and interruptive.

Travel

Travel outside of Northshore locations will be rare for this position.

Qualifications

Role Qualifications

  • Must be at least 18 years of age
  • Must have reliable transportation
  • Licensed Physician in the State of Indiana
  • CSR License in the State of Indiana
  • DEA license
     

Preferred Experience Requirements

Two years of experience working within a healthcare setting.

Minimum Education Requirements

Graduate of an accredited Medicine program

Physical Requirements

While performing the duties of this job, the employee may be required to sit and/or stand for prolonged periods, work longer than eight (8) hour shifts, and to work both day/evening/weekend shifts.  Work may include hand dexterity as well as the need to reach, climb, balance, stoop, kneel, crouch, talk, and hear.  The employee must occasionally lift and/or move up to 50 lbs.  While performing the responsibilities of the job, the employee is required to talk and hear. Specific vision abilities required by the job include close vision, distance vision, color vision, peripheral vision, depth perception, and the ability to focus.
