Job Summary

We are seeking a skilled and compassionate Physiatrist to join our growing team. In this role, you will provide comprehensive rehabilitation care for residents in skilled nursing and long-term care facilities, focusing on pain management, functional improvement, and quality of life. You will collaborate closely with primary care providers, therapists, and interdisciplinary teams to develop individualized care plans and support optimal patient outcomes.

Responsibilities
  • Conduct comprehensive evaluations of patients with functional impairments, disabilities, and chronic conditions
  • Diagnose and manage rehabilitation and pain-related conditions
  • Develop and implement individualized rehabilitation care plans in collaboration with interdisciplinary teams
  • Prescribe and oversee physical, occupational, and other therapeutic interventions
  • Perform procedures within scope of practice (e.g., joint injections, pain management techniques) as applicable
  • Communicate treatment plans, progress, and recommendations to patients, families, and facility staff
  • Monitor patient progress and adjust care plans to optimize outcomes
  • Ensure timely follow-up on treatment plans and interventions
  • Document clinical findings, care plans, and outcomes accurately in the medical record
  • Maintain compliance with HIPAA, infection control standards, and facility policies
  • Collaborate with physicians, advanced practice providers, therapists, and nursing staff to support coordinated care
  • Participate in quality improvement initiatives and support best practices in rehabilitation care
  • Travel to partner skilled nursing and long-term care facilities as assigned

Requirements

  • Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) from an accredited medical school
  • Completion of an ACGME-accredited residency in Physical Medicine & Rehabilitation (PM&R)
  • Board-certified or board-eligible in Physical Medicine & Rehabilitation
  • Active and unrestricted Physician license in the state of practice
  • Current ACLS and BLS certifications preferred
  • Prior physician experience in rehabilitation medicine, pain management, or related settings preferred
  • Experience working in skilled nursing or long-term care settings is a plus
  • Strong clinical assessment and functional evaluation skills
  • Excellent communication and interdisciplinary collaboration abilities
  • Experience providing care to the geriatric population
  • Ability to travel to multiple facilities as needed
  • Prolonged standing/walking. Assisting with patient positioning. Occasional lifting. Lifting/moving patients.
