Position Overview: 

The Arc is seeking a full-time Person-Centered Planning Coordinator.  Reporting to the Division Director, you will provide coordination and oversight of the case management and person-centered planning (PCP) functions for all Maryland Developmental Disabilities Administration (DDA) community living services, to ensure a consistent person-centered planning process throughout all programming. Duties include establishing and monitoring a master schedule of PCPs, quarterly review dates, and working closely with internal staff and Coordinators of Community Services (CCS) to ensure all quality standards are met.  Assists in accepting and/or declining services in Maryland’s Long-term Services and Support (LTSS). You will be responsible for creation, maintenance and training on any and all PCP documents to facilitate successful annual and interim meetings. You will also be responsible for training on and compliance with PCP requirements.

Key Responsibilities:

  • Oversee the process of ensuring proper preparation of all PCP documents and activities by each Community Living program, to guarantee a successful and efficient PCP meeting.
  • Serves as an advocate for all individuals supported in the Arc’s Community Living Programs to ensure their wishes and dreams are accurately represented in annual plans.
  • Train staff and oversee timely submission of Behavioral Support Plans (BSP’s).
  • Train Community Living staff and oversee proper documentation of Rights Information to the HRAC.
  • By random and scheduled reviews, ensures that all services on the PCP are aligned with current waiver services.
  • Submit all necessary paperwork for funding (DSAT or SSP) and planning (PCP) to DDA.
  • Ensure that all necessary services are funded and to ensure that all services that are delivered are identified as funded by DDA correspondence.
  • Design and implement support and teaching strategies targeted at assisting clients with reaching their outcomes, as outlined in the client's PCP
  • Provide supervision and training to Direct Support Professionals regarding the implementation of the PCP, ISP and teaching strategies
  • Collaborate with the Director of Admissions on accurate PCP and supporting documentation for the annual plan as well as Nursing supports.
  • Evaluate and revise support and teaching strategies, as necessary, to ensure progress toward goals.
  • Ability to work effectively with direct care, clinical, professional and administrative level staff and to interact in a professional and therapeutic manner with families or community agencies.
  • Participate in highly sensitive and critical Person-Centered Plan meetings.
Must Have Qualifications
  • A Bachelor’s Degree from an accredited College or University and one year of experience in providing services to adults with developmental disabilities or a high school diploma and at least three years relevant experience supporting people with I/DD in community settings. 
  • Must have demonstrated ability to develop and coordinate trainings and PCP meetings.
  • Experience interacting with a variety of disciplines including medical professionals, clinical staff, family members, education institutions and other stakeholders.
  • Must be efficient, detail oriented, process driven and ensure all necessary information is communicated appropriately and facilitate meetings.
  • Must be computer literate and have excellent oral and written communication skills.
