POSITION TITLE: OB/GYN Referral Navigator 

LOCATION: HOPE Clinic  

REPORTS TO: OB/GYN Referral Navigator Team Lead

EDUCATION: High School Diploma or GED; Associate degree preferred

WORK EXPERIENCE: 2 years or more in healthcare preferred

SALARY RANGE: $14.50-$17.00/Hour

FLSA STATUS: Hourly – Non-Exempt

POSITION TYPE: Full-Time

JOB SUMMARY:

Working with medical and clinical providers, the Referral Navigator will coordinate outgoing referrals for patients of Asian American Health Coalition of the Greater Houston Area (AAHC) DBA HOPE Clinic with outside providers and insurance companies. Works hand-in-hand with patients, peer referral specialists, and clinical staff to provide a quality referral experience to outside specialists and facilities. 

MAJOR DUTIES & RESPONSIBILITIES:  

  • Provide referral details and appointment arrangements to patients needing specialist or external medical visits;
  • Manage follow-up workflows to ensure that patient visits with specialists are completed and referral notes are obtained from external providers;
  • Stay up to date with external referral sources and provide appropriate referral options for patients based on services needed;
  • Facilitate DME orders between providers and companies 
  • Coordinate surgery appointments requested by assigned provider;
  • Coordinate inductions & c-sections with delivery hospitals 
  • Work together with other team members as backups when needed;
  • Send out OB Charts to delivery hospital one-month prior estimated due date;
  • Initiate pre-authorization or pre-certification for hospital procedures;
  • Verify patient insurance and information is acceptable by partnering institutions;
  • Service and respond to customer and patient questions, issues and/or concerns;
  • Securely and accurately document all patient information;
  • Submit Agency forms and documentation in a timely manner. (Improvement activities, response to memorandums, travel reconciliation, supply requests, leave requests, monthly in-service);
  • Positively contributes to staff morale and corporate culture;
  • Ensure that work assignments and information gathered from day-to-day work is not shared with anyone and protect all patient and agency information. Is knowledgeable of and maintains HIPAA standards of privacy and confidentiality;
  • Ensure that all patients receive the best possible care;
  • Attend on-site/off-site community engagement activities and on-site/off-site clinic events as needed; 
  • Perform other duties as assigned to support HOPE Clinic’s Mission, Vision and Values.

Requirements

QUALIFICATION REQUIREMENTS:

  • Minimum of one year experience in coordinating referrals to outside providers or relevant experience with an insurance company
  • Insurance and medical experience preferred
  • Microsoft Office
  • Must be able to utilize the Internet
  • Strong written and oral communication skills
  • Strong customer service skills
  • Must be detailed oriented and able to handle multi-tasks
  • Office equipment (e.g., computer, copier)
  • Detail oriented
  • Able to handle multiple tasks, complexity, and diversity of patients

EDUCATION and/or EXPERIENCE:

High School diploma or GED is required; Associates Degree is preferred. Experience in the medical field and referral/health insurance is preferred. 

OTHER SKILLS and ABILITIES:

  • Fluently bilingual in another language served by HOPE Clinic as well as English is required.
  • Above average skills in language ability as well as public speaking and writing. 
  • Must have good transportation and a valid Texas Driver’s license.
