Position Overview
Under close supervision, performs moderate to complex nutrition case management for medically high-risk individuals. Provides physician-prescribed dietary counseling and Medical Nutrition Therapy (MNT) services in clinic and community settings. Reports to the Nutrition Services Supervisor or Director of Nutrition.
Minimum Requirements
Licensure (Required):
Registered Dietitian (RD) licensed by the Kentucky State Board of Licensure and Certification (KRS 310.021); or
Certified Nutritionist (CN) licensed by the Kentucky State Board of Licensure and Certification (KRS 310.031).
No prior experience required.
Essential Duties & Responsibilities
Assess nutrition status and develop individualized nutrition and MNT care plans.
Provide individual and group nutrition counseling.
Implement physician-prescribed dietary regimens.
Interpret nutrition research and provide consultation to healthcare staff.
Participate in on-the-job training and competency development activities.
Maintain documentation in accordance with agency policy and WIC & Nutrition Manual guidelines.
Obtain and maintain National Provider Identification (NPI) number for MNT reimbursement.
Assist with community outreach, program development, and in-service education.
Perform other duties as assigned.
