SummaryProvides medical, regulatory oversight, and support to the residential nursing team. Responsible for overseeing all health care services to individuals served in our residential program. Incumbent must comply with applicable regulatory requirements and standards in the performance of nursing duties.
Essential Job Functions- Effectively recruits, hires, trains, directs, counsels, disciplines, and evaluates performance of residential Licensed Practicing Nurses (LPNs) and any additional registered nurses who are hired.
- Provides direct supervision, support, and guidance to LPNs. Assigns direct nursing care assignments and caseloads to ensure completion of documentation and reviews. Approves all documentation completed by LPNs.
- Serves as consultant for Shared living coordinators and program directors.
- Develops, implements, and monitors residential nursing policies and procedures as well as makes recommendations for improvement in collaboration with the director and interdisciplinary teams.
- Conducts nursing assessments and ensures that service delivery is in compliance with regulations and healthcare protocols.
- Collaborates with day program nurses and other licensed clinicians to ensure continuity of care.
- Acts as medical representative in situations of medical care, collaborating with internal and external stakeholders.
- Collaborates with interdisciplinary teams across all program settings as needed.
- Reviews and monitors Medication Administration Program (MAP) policies to ensure medication administration practices are compliant with all current regulatory requirements.
- Ensures that all nursing team members remain current with required medical licensures, certifications, and trainings, including oversight of CAN certification process.
- Monitors and assists with the coordination of each participant’s medical needs with external health care professionals, including when the participant is inpatient or receiving health care from an outside source.
- Supervise and supports the nursing team in the oversight and coordination of all therapy treatments for individuals as ordered by external health care professionals.
- Analyzes critical incident reports and data for trends, develops and implements corrective action plans as necessary.
- Advises the Program Directors, clinicians, group home managers and direct support staff of any medical problems that may hinder a participant’s progress or that may be aggravated by activities/daily routines and makes recommendations for improvement of medical care.
- Oversees a medical auditing system to monitor quality improvement initiatives.
- Serves as the medical representative member of the agency PBS leadership team.
- Collaborates with the residential team on the intake process for new referrals.
- Collaborates with clinicians and other members of the residential team on the development of supportive and protective (S&P) devices, medical treatment plans, and as needed (PRN) support strategies, including presenting at Human Rights Committee (HRC), Peer Review Committee (PRC), and program-specific Safety Committee as needed.
- Responsible for oversight and completion of staff training regarding medical and nursing, including new employee orientation training, annual nursing training, and first aid and CPR.
- Participates in manager’s, clinical, and staff meetings as needed to review health care needs and global health care planning with all staff.
- Responsible for creation and maintenance of psychotropic medication treatment plans, supportive and protective devices, and PRN support strategies, including presentation at Human Rights Committee as required.
- Serves as mandated reporter as required by the DPPC.
- Performs direct support related tasks in emergencies or as requested by Residential Director.
- Fosters and maintains positive relationships with funding sources, involved families, and nursing staff teams across all agency settings. Manage family, individual, staff, and community complaints quickly and proactively; informs the Clinical Director of major and/or unresolved issues.
- Ensures that human rights, safety, and respect for individuals supported by the agency are safeguarded in all medical interventions and interactions by program nursing teams.
- Ensures the proper documentation of emergency events is completed by all program nursing teams.
- Position will require on-call, weekend, and evening responsibilities as needed.
Residential Licensed Practical Nurses (LPNs)
QualificationsEducation and Experience- Registered Nurse, registered with the Massachusetts Board of Registration in Nursing.
- BSN preferred but not required.
- 5+ years experience in a community health setting.
- Prior experience working with developmentally disabled adults.
