What You'll Do
As a Home Care Coordinator, you’ll be an important part of the team behind the scenes—helping connect patients, families, referral sources, and clinicians to ensure care is coordinated smoothly and efficiently.
You’ll be responsible for:

Coordinating communication between physicians, referral sources, patients, caregivers, and our clinical team

Supporting scheduling and administrative processes to keep patient care moving efficiently

Working closely with clinicians to help ensure timely, compassionate, and well-coordinated care
-
Using your clinical knowledge to understand patient needs and support appropriate care coordination
-
Helping resolve questions and issues to create a positive experience for patients, families, and healthcare partners

Contributing to a collaborative team environment focused on delivering exceptional home care
