At Advanced Medical Home Care, we’re looking for a Licensed COTA or PTA to join our Home Care Coordination Team. This is an in-office, Monday–Friday position that allows you to use your clinical knowledge, communication skills, and understanding of patient care in a new and rewarding way.

What You'll Do

As a Home Care Coordinator, you’ll be an important part of the team behind the scenes—helping connect patients, families, referral sources, and clinicians to ensure care is coordinated smoothly and efficiently.

You’ll be responsible for:

• Coordinating communication between physicians, referral sources, patients, caregivers, and our clinical team

• Supporting scheduling and administrative processes to keep patient care moving efficiently

• Working closely with clinicians to help ensure timely, compassionate, and well-coordinated care

• Using your clinical knowledge to understand patient needs and support appropriate care coordination

• Helping resolve questions and issues to create a positive experience for patients, families, and healthcare partners

• Contributing to a collaborative team environment focused on delivering exceptional home care

Who We’re Looking For

We’re looking for a Licensed COTA or PTA who is ready to bring their clinical experience into a collaborative, fast-paced coordination role.

What You’ll Need

• A current Minnesota PTA or COTA license in good standing

• Knowledge of medical terminology and healthcare processes

• Strong communication, critical-thinking, and decision-making skills

• Excellent attention to detail and organizational abilities

• Technological proficiency and a commitment to accuracy

• Consistent attendance and reliability
