Job Description:

Job Summary

Facilitates timely access to clinical services and resources by coordinating care and supporting navigation through the healthcare system. Identifies patient needs and connects them with appropriate services. Communicates effectively to ensure understanding and satisfaction. Promotes positive patient experiences and outcomes through advocacy and support.


Essential Functions

• Identifies patient needs related to access to healthcare.

• Coordinates and facilitates patient appointments, referrals, and follow-up care.

• Facilitates charity care processes for uninsured patients.

• Mentors patients to understand and comply with treatment goals, reviewing progress regularly.

• Collaborates with healthcare team members regarding patient care.

• Serves as a resource for hospital and community-based services information.

• Manages processes to identify patients for navigation, including screenings, referrals, procedures, and treatment.

• Performs other duties as assigned.


Education Requirement

Bachelor’s degree in public health, social work, or a related field


Experience Requirement

1-2 years of experience in healthcare, social services, or a relevant area


An equivalent combination of education and experience may be considered.

All experience must be paid and in the same related field.

Part-time and PRN experience will be prorated based on hours worked per week.

Volunteer work and internships for academic credit are not counted.

Physical Demands

This position requires intermittent sitting at a computer workstation for extended periods of time; performing tasks with repetitive motions; intermittent standing or walking with objects weighing up to 10 pounds; occasional standing or walking with objects up to 25 pounds; and occasional lifting, pushing, and/or pulling objects weighing up to 50 pounds. Also involves some exposure to risk of back injury from moving, lifting, or positioning equipment or materials, occasional risk of injuries from use of equipment, and occasional exposure to cuts/punctures and bloodborne or airborne pathogens.

Essential Duties and Responsibilities

Patient Navigation

  • Identify patients eligible for lung cancer screening.
  • Conduct eligibility assessments based on current screening guidelines.
  • Perform and document shared decision-making visits (when applicable).
  • Educate patients regarding benefits, risks, limitations, and expectations of LDCT screening.
  • Coordinate scheduling of LDCT examinations.
  • Monitor completion of ordered screenings.
  • Notify patients of results and next steps.
  • Coordinate annual screening reminders and recalls.
  • Assist patients in overcoming barriers to care.

Clinical Coordination

  • Review all LDCT reports.
  • Track Lung-RADS classifications.
  • Ensure appropriate follow-up based on Lung-RADS recommendations.
  • Coordinate referrals to pulmonology, thoracic surgery, oncology, and other specialties.
  • Manage incidental pulmonary nodules and significant incidental findings.
  • Collaborate with radiologists and pulmonologists regarding abnormal findings.
  • Coordinate multidisciplinary care when lung cancer is suspected.

Smoking Cessation

  • Assess tobacco use.
  • Provide evidence-based tobacco cessation counseling.
  • Refer patients to smoking cessation resources and programs.
  • Document counseling provided.

Program Management

  • Maintain the lung cancer screening registry.
  • Track compliance with annual screenings.
  • Monitor follow-up compliance.
  • Ensure adherence to CMS and accreditation requirements.
  • Participate in quality improvement initiatives.
  • Collect and analyze program metrics.
  • Assist with annual program reporting.

Provider Education

  • Educate primary care providers on screening eligibility.
  • Develop educational materials.
  • Promote appropriate referral practices.
  • Serve as a clinical resource for lung cancer screening guidelines.

Community Outreach

  • Participate in community education events.
  • Promote awareness of lung cancer screening.
  • Build relationships with referring providers and community organizations.

Documentation

  • Maintain complete and accurate documentation in the electronic medical record.
  • Track patient outcomes.
  • Maintain registry data.
  • Document patient education and tobacco counseling.


Qualifications

Education

  • Graduate of an accredited nursing program.
  • Bachelor of Science in Nursing (BSN) preferred.
  • Master of Science in Nursing (MSN) preferred.

Licensure

  • Current Registered Nurse (RN) license.
  • Basic Life Support (BLS) certification.

Preferred Certifications

  • Oncology Certified Nurse (OCN)
  • Certified Tobacco Treatment Specialist (CTTS)
  • Nurse Navigation certification (preferred)

Experience

  • Three or more years of nursing experience.
  • Experience in pulmonology, oncology, thoracic surgery, radiology, or critical care preferred.
  • Experience with patient navigation preferred.


Knowledge and Skills

  • Knowledge of lung cancer screening guidelines.
  • Understanding of Lung-RADS reporting.
  • Knowledge of pulmonary nodules and incidental findings.
  • Strong patient education skills.
  • Excellent communication and interdisciplinary collaboration.
