The NAVIGATE program at M Physicians is affiliated with the University of Minnesota Department of Psychiatry and Behavioral Sciences, a leader in first episode psychosis care and research. The NAVIGATE program started in 2017 with the support of grant funds from the Minnesota Department of Human Services. NAVIGATE is an evidence-based coordinated specialty care program to treat first episode psychosis in schizophrenia spectrum disorders. This interdisciplinary team-based approach is resiliency- and recovery-focused. Team members work with individuals ages 15 to 40 years old, and their natural support persons, to help them understand their illness and develop skills that help them get back on track and live rich and meaningful lives. NAVIGATE involves a number of different interventions including medication management, individual resiliency training (IRT), help getting back to work or school (Supported Employment and Education or SEE), peer support, case management and care coordination, and family support and education. NAVIGATE team members are highly skilled at providing both clinical and non-clinical support and services to individuals and families impacted by a first episode of psychosis, whether that be in the clinic, community, or an individual's home. Team members are highly collaborative, attend weekly team meetings to discuss patient care, and receive one-to-one supervision from the program’s director.

What you will do as a NAVIGATE Program Coordinator:
  • Attend all NAVIGATE team meetings, record meeting minutes, and track team member action items.
  • Develop and maintain up-to-date program policies and workflows.
  • Assist with coordinating and onboarding new team members and learners.
  • Support team members with implementing and maintaining special projects, events, or initiatives; e.g., groups, workshops, program marketing and community education.
  • Answers, coordinates and facilitates responses to program referrals and requests for more information.
  • Coordinate receipt of needed consents for services and releases of information, and obtains internal and external records whenever possible.
  • Secures data/facts/information to assure appropriate scheduling and insurance reimbursements.
  • Supports team members with patient/family care coordination needs including helping set program expectations with patients and families
  • Monitors NAVIGATE visit frequency and assist with scheduling appointments and transportation to/from appointments.
  • Monitor progression of NAVIGATE services from enrollment to graduation/discharge, keeping the team apprised of the length of the patients enrollment, facilitating the graduation/discharge process
  • Uses appropriate techniques for handling urgent patient issues including appropriate use of urgent Evaluation appointments per clinic protocol. 
  • Answers the mental health NAVIGATE phone line as possible
  • Maintains timely and accurate notes in Epic and other systems
  • Communicates effectively with other members of the health care team, including summarizing complex patient situations
What you will need:
  • Bachelor's Degree in Psychology, Social Work, or related Social Sciences field
  • 2+ years of experience working with individuals impacted by mental illness, including but not limited to psychosis

*This role may require the employee to be available beyond traditional office hours. The person in this positions needs to be comfortable working directly in the community with clients, including home and community visits*

Location: St. Louis Park Clinic - 5775 Wayzata Blvd Suite 200 St. Louis Park, MN 55416

  • This is a hybrid position with work locations including in clinic (St. Louis Park Psychiatry Clinic), in the community (i.e. coffee shop, library, patient home), and from home.
  • Percentage of time spent working in clinic or in the community is based on patient need/preference.

Hours: 40 hours/ week

Compensation:

53,570.00 - 77,672.00 USD Annual

Skills Required
