JOB ACCOUNTABILITIES:
CLINICAL PROGRAM COORDINATION.
• Collect and track patient care quality metrics, advising on gaps in certification measures
• Identify and implement protocols designed to satisfy the accreditation and certification requirements (as applicable) of the program
• Real time monitoring of patient care to ensure clinical practice guidelines and timelines are followed throughout the continuum of care
• Provides input to patient assessment processes and treatment care plans for the patient population and suggests/drafts revisions to or devises new policies and procedures that guide and support the provision of services.
• Responsible for the implementation, integration and clinical support for quality assurance, performance, performance improvement and risk management activities of the program
• Acts as a resource for members of the interdisciplinary team on care and protocols to promote positive outcomes for patients.
• Coordinates and supervises nursing standards of practice for patients and manages data reporting including program survey participation, monitoring of key performance measures, staff competencies and continuing education as well as protocol/ guidelines oversight
EDUCATION.
• Work closely with patients, families, physicians, and members of the outpatient and inpatient nursing staffs, and other health care team members in formulating instructional patient care and teaching plans
• Uses knowledge of specific clinical needs when developing educational materials.
• In conjunction with nursing staff, medical staff, and management, establishes a clinical program education program for orientation and continuing education.
• Contributes to community awareness through outreach program involvement and or development.
PATIENT CARE.
• Partners with peers, other healthcare providers, and management to effectively streamline patient workflow, improve patient outcomes, and provide the highest care quality
• Communicates patients’ plan of care to interdisciplinary care teams and between departments.
• Provides inpatient and outpatient education in the form of individual consults, conducts classes and support groups for patients and the community at large.
• Provide direct patient care and/or respond to code as needed, on an irregular basis
LEADERSHIP.
• Acts as a resource for colleagues with less experience; may lead small projects with manageable risks and resource requirements.
• Actively participates or leads data analysis and develops recommendations and action plans.
• Solves complex problems; takes a new perspective on existing solutions; exercises judgment based on the analysis of multiple sources of information.
SAFETY.
• Delivers safe and effective patient care in compliance with established hospital policies and procedures, as well as local, state and federal regulations to provide the highest level of patient care.
• Upholds the confidentiality of all patient protected health information (PHI) and protects all PHI from accidental, intentional or inappropriate disclosure.
• Remains current in advancements, trends, and best practices related to acute care and area of specialty and in professional nursing practice.
WHEN SUPPORTING A CENTER OF EXCELLENCE BARIATRIC PROGRAM
Ensure timely submission of outcome data to the Metabolic and Bariatric Surgery Quality Improvement (MBSAQIP) registry
Collaborates with leader to manage program pathways, protocols, oversight standard compliance, and engaging stakeholders in the work of process/quality improvements
Assist in program development, managing the accreditation process, ensuring continuous compliance with MBSAQIP standards, maintaining relevant pathways and protocols, patient education, monitoring outcomes data and collection, quality improvement initiatives, and the education of relevant staff with a focus on metabolic and bariatric patient safety
Serve as the liaison between facility and all metabolic and bariatric proceduralists and general surgeons providing call coverage
Facilitates and attends committee meetings as required by MBSAQIP standards
EDUCATION:
Graduate of an accredited school of nursing

Pay Range is $189,300.80 to $249,870.40 / annual salaryThe compensation range may vary based on the geographic location where the position is filled. Total compensation considers multiple factors, including, but not limited to a candidate’s experience, education, skills, licensure, certifications, departmental equity, training, and organizational needs.
