Paradise Creek Health and Rehabilitation of Cascadia is looking for dedicated and skilled Licensed Practical Nurses (LPNs) and Registered Nurses (RNs) to join our team! Key Responsibilities
- Administer medications and treatments as prescribed
- Monitor residents' health status and report any changes to the healthcare team
- Develop and implement individualized care plans in collaboration with residents, families, and the interdisciplinary team
- Supervise and support Certified Nursing Assistants (CNAs) in delivering resident care
- Assist with admission, discharge, and transfer processes
- Ensure accurate and timely documentation in accordance with state and federal regulations
- Provide emotional support to residents and their families
- Participate in ongoing education and training to maintain clinical competence
Qualifications
- Current LPN or RN license in the state
- Strong clinical and critical thinking skills
- Excellent communication and interpersonal abilities
- Ability to work effectively as part of a team
- Compassionate and patient-centered approach to care
- Previous experience in long-term care, geriatric, or rehabilitation settings is a plus
- Ability to manage multiple tasks and prioritize effectively

Requirements
Education
Graduate of accredited LPN School of Nursing.
Licenses/Certification
Current valid LPN license in the state employed.
Valid CPR certification.
Experience
Six months experience in a long-term care environment preferred.
