Home Health Services of Virginia is seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our team. This position offers a Monday through Friday schedule from 4pm to 10pm with no holiday shifts, providing you with a consistent and balanced work-life routine. The LPN will provide quality nursing care to our patients in Henrico County, VA, ensuring their comfort and well-being in a home health setting.
Responsibilities- Provide skilled nursing care and treatment to patients in their homes according to physician prescriptions and nursing care plans.
- Monitor patients’ health status and report any changes to the supervising nurse or physician promptly.
- Administer medications and injections as prescribed.
- Maintain accurate and detailed patient records and documentation.
- Educate patients and their families regarding health management and care plans.
- Collaborate with healthcare team members to coordinate patient care effectively.
- Valid LPN license in the state of Virginia.
- Previous experience in home health care or related nursing field preferred.
- Strong communication and interpersonal skills.
- Ability to work independently and manage time effectively during home visits.
- Reliable transportation to travel within Henrico County, VA.
- Competitive hourly pay of $32.00 - $37.00, paid bi-weekly.
- Monday through Friday schedule from 4pm to 10pm with NO HOLIDAYS.
