Provide licensed practical nursing care in patients' homes: follow and reassess care plans, perform skilled nursing services, complete clinical notes, work scheduled shifts, and perform additional assigned duties.

- Must hold a current license as a Licensed Practical Nurse or Registered Nurse from the Virginia Board of Nursing.

Job Duties:
- Complete patient care duties.
- Follow patient plan of care and reevaluate as necessary.
- Work scheduled shift to perform patient duties.
- Complete clinical nurse notes in their entirety.
- Complete skilled care services as applicable.
- Additional duties as assigned.
Skills Required
- Current license as a Licensed Practical Nurse or Registered Nurse from the Virginia Board of Nursing
- Experience working in a home care/home health agency
