Position Summary
Under the direction of a Registered Nurse, the LPN will provide direct patient care, meeting both psychological and physical needs of the patients in accordance with physicians’ orders, AOA, OSHA, nursing and department policies. The LPN fulfills these duties to contribute to the continuity and quality of care that fosters the best interest and well being of all patients and their families.

Essential Functions
Under the supervision of the RN, assists in the implementation of the patient’s treatment plan.
Communicates with RN and reports any changes, to medical orders, radiological results, laboratory results, or patient status.
Adheres to facilities policy and procedures.
Participates in hospital wide performance improvement and patient safety initiatives.
Participates in nursing and hospital committees as appropriate. Attends at least 70 % of department staff meetings.
Demonstrates strong customer service skills, and professional behavior when dealing with all individuals and groups, both internally and externally.
Demonstrates respect and positive communication skills when interacting with all individuals and groups, both internally and externally.
Functions as a patient advocate through collaboration and communication of patient needs.
Utilizes manual and automated information systems to improve patient safety.
Under supervision of RN, conducts and documents a thorough assessment of each patient’s medical status per unit guidelines and throughout the patient’s course of treatment.
Documents pertinent information in an appropriate format and according to hospital policy.
Maintains position specific skills and competencies as required on annual basis.
Attends educational programs appropriate to discipline in order to maintain clinical competency.
Functions as patient advocate through education to the patient and the family including health promotion.
Reviews, updates and implements multidisciplinary plan of care in collaboration with the RN.
Responds to patient care needs in a timely fashion, including, but not limited to, call bell response, verbal requests, and bed exit alarms.
Maintains adherence to patient rights.
Administers prescribed treatments, medications, and therapies within the scope of practice.
Demonstrates cost effectiveness by using supplies judiciously, anticipating needs for special supplies and equipment, and making recommendations for change.
Complies with established personal protective equipment requirements necessary for protection against exposure to blood and other potentially infectious body fluids, chemical disinfectants, radiation, antineoplastic/cylotoxic agents and other hazardous substances.
Performs other duties as assigned

Reporting Relationships
Reports to: Charge Nurse / Resource Nurse / Clinical Coordinator / Nurse Manager.
Supervises: Nursing Assistant
Qualifications
Graduate of a State Board approved program of practical nursing. Holds a current Pennsylvania state license as licensed practical nurse. Certified in basic CPR.
