We’re looking for a compassionate, dependable Hospice LPN to join our team and provide exceptional, person-centered care in the community.

What You’ll Do
  • Provide skilled nursing care to hospice patients in their homes and other care settings.

  • Monitor patient conditions and communicate changes to the hospice care team.

  • Administer medications and treatments as directed by the plan of care.

  • Educate and support patients and families with compassion, patience, and respect.

  • Work collaboratively with RNs, providers, social workers, chaplains, aides, and other members of the interdisciplinary team.

  • Help patients remain comfortable and supported while honoring their goals and preferences.

  • Complete accurate clinical documentation and follow hospice standards of care.

What We’re Looking For
  • Current LPN license in Maine.

  • Strong clinical skills and sound judgment.

  • Excellent communication and organizational skills.

  • A compassionate, respectful approach to patients and families.

  • Ability to work independently while being an engaged member of a collaborative team.

  • Hospice, home health, or palliative care experience is valued, but we’re also interested in LPNs who are passionate about learning hospice care.
