In this role, you’ll guide and support a dedicated team delivering comprehensive case management services to fathers, connecting them to community resources, natural supports, and opportunities for growth. You will play a key role in ensuring high-quality services, strong outcomes, and continuous program improvement.

This position blends supervision, case management knowledge, data-driven decision-making, and community collaboration.

TITLE: Lead Case Manager (Champion)
CLASSIFICATION: Exempt
SALARY RANGE: $50-$52k

Position Summary: This is a supervisor level position responsible for leadership and direct oversight of the case management services within the FRANC program. FRANC case management services provide support services for fathers utilizing a broad range of community services and natural support systems. This position reports to the FRANC Manager. Specific responsibilities include, but are not limited to:


ESSENTIAL DUTIES:

1. Assist in developing, implementing, and monitoring quality assurance processes.

2. Maintain data systems for collection, aggregation, and reporting, ensuring compliance with funding contracts and accurate tracking of progress. Generate reports to monitor staff and program performance.

3. Use data to evaluate and improve program outcomes, following the PDSA model. Assist with continuous improvement in service delivery, documentation, and staff development.

4. Supervise and train case management staff, conduct assessments, and connect fathers to services. Monitor progress and foster relationships with external agencies to support fathers.

5. Research and deliver case management services within the FRANC’s program framework when needed.

6. Build relationships with community partners and participate in outreach efforts. Contribute to case reviews with the interdisciplinary team.

7. Complete accurate data collection and reporting, attend staff meetings, and participate in ongoing training and community planning.

8. Participate in appropriate agency, department, and community planning teams.

9. Performs other related duties as assigned.

Qualifications

REQUIREMENTS:

1. Minimum bachelor’s degree in social work, human services or related field.

2. Minimum of 3 years’ experience working with fathers and father-figures and 2 years’ experience supervising staff.

3. Organize and prioritize work assignments; make decisions and independent judgments; determine the appropriate course of action in emergency or stressful situations; and recognize reportable events.

4. Computer skills with proficiency in Microsoft Office (Excel, Word, Outlook), Microsoft 365 web-based applications and using internet search engines and other online research tools.

5. Ability to work in the office and in the community.

6. Available to work some evenings and Saturdays according to participants' needs and the needs of the program.

7. Professional, self-directed, follow-through on projects and prompt responsiveness to internal and external stakeholders.

8. Good interpersonal skills, team-oriented, customer service focused enjoys working with others.

9. Strong verbal communication skills and demonstrated ability to write clearly and persuasively.

10. Detail oriented, with excellent organization skills required.

11. Light physical demands: Regularly required to sit; use hands to finger, handle or feel; reach with hands and arms and talk and hear. Ability to walk, stand, climb stairs, kneel, bend, reach, and manipulate objects; lift and move materials up to 10 pounds on a regular basis; infrequently required to lift or move materials weighing up to 20 pounds.

12. Able to travel in the communities served; must have a reliable car, active auto insurance, and valid Florida driver’s license.

13. Must be able to successfully complete a Florida Level 2 criminal background check, motor vehicle records check and drug screening.
