General Responsibilities

  1. Respond to inquiries via phone, email, in-person, or online platforms.
  2. Assess client needs and determine appropriate services or resources.
  3. Provide accurate information and referrals to internal departments or external agencies.
  4. Maintain up-to-date knowledge of community resources, programs, and eligibility requirements.
  5. Document all interactions and maintain confidential client records in accordance with organizational policies.
  6. Follow up with clients to ensure successful connection to services.
  7. Assist participants in completing Benefit Access Program applications.
  8. Assist individuals with understanding and enrolling in Medicare and prescription drug plans.
  9. Coordinate transportation services by enrolling participants and supporting ride scheduling.
  10. Provide Options Counseling in alignment with funder guidelines and program standards.
  11. Collaborate with team members and community partners to improve service delivery.
  12. Participate in training sessions and professional development activities.
  13. Serve as an agency ambassador in the community, attending fairs and conducting presentations.
  14. Other duties as assigned within the guidelines of this position.

Requirements

  1. Associate’s degree required; Bachelor’s degree preferred in social work or social sciences.
  2. Minimum of one year of experience with community resources for older adults and/or adults with disabilities.
  3. AIRS Certification preferred or must be obtained within 18 months of hire.
  4. SHIP Certification preferred or must be obtained within six months of employment.
  5. Ability to relate well to older individuals and persons with disabilities, respecting their privacy and dignity.
  6. Strong problem-solving skills and adherence to a no-wrong door policy.
  7. Ability to handle sensitive information with discretion and professionalism.
  8. Ability to work independently and manage multiple tasks.
  9. Proficiency in Microsoft Office applications (Outlook, Word, Teams).
  10. Passage of complete background clearance and any other required testing.
  11. Adhere to the Agency’s Code of Ethics and conduct all responsibilities in accordance with the standards outlined in the Employee Handbook. 
  12. Valid driver’s license, reliable transportation, and proof of liability insurance.

Skills Required
