Position Summary: 

The Clinical Liaison is to be accountable for census, business development, and sales-related activities for all designated assigned hospital accounts and additional external referral sources within the facility market. This is an external position that includes daily rounding activities with assigned hospital accounts, focused on census development, referral growth, and promoting efficient and effective transitions of care for the Long Term Acute Care Hospital. 


Core Responsibilities:

  • Primary sales and marketing representative for managing all primary hospital referrals/designated referral source referrals and ensuring a smooth discharge process for patients to post-acute care facilities .
  • Develop strong relationships with key hospital personnel and identify new opportunities for alignment, partnering, and growth.
  • Onsite referrals to determine if they are clinically appropriate for the care setting and round regularly to coordinate the optimal time for transfer.
  • Engage in regular case finding at assigned hospitals to identify patients not previously identified who would benefit from LTAC services.
  • Provide tours for families to promote the facility and services.
  • Work with facility marketing team to develop monthly plans and strategies.
  • Oversee the admissions process to ensure that referral/intake process is user friendly 24 hours per day; 7 days per week by working with hospital and designated facility staff.
  • Assist in planning and organizing marketing strategies and programs designed to meet census and payer mix goals.
  • Prepare weekly reports on activity, competitor/referral sources.
  • Prepare a semi-annual competitive review analyzing the market, including changes in competitive and referring facilities as well as the community.
  • Assures that the admission process is seamless and positive, including but not limited to, identifying all necessary device/equipment and immediate medical needs necessary to ensure a smooth transition to the facilities.
  • Provides regular feedback to referring providers regarding outcomes of referred patients. 
  • Other duties as requested and assigned.

Minimum Qualifications:

  • Minimum of 2 years’ experience case management, liaison, clinical marketing experience.
  • Respiratory Therapist or Registered Nurse with critical care or LTACH experience is required
  • Familiarity with the admissions process at a sub-acute, SNF or rehab facility.
  • Ability to assess clinical needs of potential facility admissions to assure that the patient will benefit from treatment at the facility and that the facility is eligible for reimbursement for the episode of care. 
  • Must have the ability to establish good working relationships with social workers, nurses, physicians, and case managers in assigned hospitals and other medical facilities.
  • Computer experience and understanding of hospitals EHR system is preferred.

Skills Required

  • At least 2 years of experience in case management, liaison, or clinical marketing
  • Registered Nurse or Respiratory Therapist credentials
  • Critical care or LTACH experience
  • Familiarity with admissions processes at a sub-acute, skilled nursing, or rehabilitation facility
  • Ability to assess clinical needs and facility reimbursement eligibility for potential admissions
  • Ability to establish relationships with social workers, nurses, physicians, and hospital case managers
  • Computer experience and familiarity with hospital EHR systems

Because LTACHs are able to focus their energy and resources at a limited number of specialized treatments with minimal overhead, they are able to provide more cost-effective care than if these same patients were kept in acute care facilities. What types of patients are seen in LTACHs? Under Medicare, the patient must need more than 25 days of hospitalization. The average length of stay of a person in an LTACH is approximately 30 days. The types of patients typically seen in LTACHs include those requiring: ■ Prolonged ventilator use or weaning ■ Ongoing dialysis for chronic renal failure ■ Intensive respiratory care ■ Multiple IV medications or transfusions ■ Complex wound care/care for burns
