As a Homecare Operations Coordinator, you will be the first point of contact for participants seeking Consultation Services and play a vital role in creating a seamless, positive experience throughout the intake and onboarding process.
In this role, you will partner with participants, Lead Agencies, service providers, and internal team members to coordinate consultation services, manage documentation, maintain accurate records, and ensure timely communication across stakeholders. This position is ideal for an organized, service-oriented professional who enjoys building relationships, solving problems, and making a meaningful impact behind the scenes.
Main Duties
- Serve as the primary point of contact for participants, families, providers, and agency partners regarding Consultation Services.
- Provide education and guidance on consultation processes, program requirements, and next steps.
- Review assessments and supporting documentation received through Minnesota DHS systems.
- Coordinate participant intake activities and ensure all required documentation is obtained and accurately maintained.
- Schedule and manage initial consultation appointments between participants and assigned Consultation Advisors.
- Accurately enter, update, and maintain participant information in internal databases and systems.
- Respond to inbound and outbound calls, emails, and inquiries in a professional, timely, and customer-focused manner.
- Collaborate with Consultation Advisors, Lead Agencies, and service providers to support a smooth and efficient participant experience.
- Monitor participant files for completeness, accuracy, and compliance with program and organizational requirements.
- Document participant interactions, communications, and activities in client management systems.
- Assist individuals with communication barriers by utilizing appropriate resources, accommodations, and support tools.
- Maintain confidentiality and ensure compliance with HIPAA, ADA, and organizational privacy policies while performing all job responsibilities.
- All other duties as assigned.
Hours: Full-time 40 hours/week, Monday – Friday (Non-Exempt)
Location: Minnetonka
Compensation: $20-$22 per hour
Requirements
- One year of in person or phone-based customer service experience required.
- Associates degree preferred.
- Experience providing direct services or experience coordinating own services preferred.
- Excellent oral and written communication skills.
- Demonstrate creativity, critical thinking, and problem-solving skills in varying environments.
- Ability to meet deadlines and leverage resources to deliver person-centered services within deadlines.
- Ability to handle numerous tasks simultaneously while maintaining strong attention to detail.
- Maintain a high standard of professional courtesy, flexibility, confidentiality, while working independently as a team member.
- Intermediate computer skills including internal office and organizational software (MS Office/Excel), and the ability to gain proficiency with Asura specific systems and navigate various online databases.
- Must pass a background study with the Minnesota Department of Human Services.
