Heart Failure Specialist Pharmacist and Independent Prescriber
ATTR-CM Amyloidosis Support Programme
Part-time – 3 days a week (remote with 1 day a week onsite in London)
We are seeking an experienced Heart Failure Specialist Pharmacist and active Independent Prescriber to support an innovative ATTR-CM Amyloidosis Support Programme delivered in partnership with the National Amyloidosis Centre at the Royal Free Hospital.
This is an excellent opportunity for a GPhC-registered pharmacist with specialist heart failure experience, cardiovascular independent prescribing expertise and confidence delivering remote clinical consultations. You will work closely with the National Amyloidosis Centre, local cardiology teams, primary care, community services and homecare providers to support high-quality, patient-centred care.
What you will be doing
As a Heart Failure Specialist Pharmacist, you will deliver specialist clinical pharmacy and independent prescribing support for patients with ATTR-CM. You will run structured virtual clinics, review treatment response, optimise guideline-directed medical therapy and provide clear education to patients, carers and clinical colleagues.
What we are looking for
We welcome applications from pharmacists who can demonstrate specialist heart failure experience, active independent prescribing practice and a collaborative approach to multidisciplinary care.
Essential requirements
- MPharm or equivalent qualification and current registration with the General Pharmaceutical Council.
- Active Independent Prescriber annotation on the GPhC register, currently used within clinical practice.
- Postgraduate clinical pharmacy qualification, such as a Diploma or MSc, or an equivalent specialist clinical portfolio.
- At least 3 years’ post-registration experience as a Heart Failure Specialist Pharmacist in secondary care cardiology or a dedicated heart failure service.
- Experience titrating guideline-directed medical therapy for heart failure in a specialist clinic setting.
- Recent independent prescribing experience within a cardiovascular scope of practice.
- Experience delivering remote or virtual consultations using digital platforms.
- Strong knowledge of heart failure pathophysiology, evidence-based management, medicines optimisation and pharmacovigilance.
- Confident clinical assessment, risk stratification, escalation and shared decision-making skills.
- Clear, empathetic communication skills with patients, carers and multidisciplinary clinical teams.
Desirable experience
- Experience supporting patients with cardiac amyloidosis, ATTR-CM, AL amyloidosis or other rare cardiovascular conditions.
- Knowledge of disease-modifying therapies
- Experience with homecare medicines services or specialised commissioning pathways.
- Experience contributing to service evaluation, audit, quality improvement or research.
- Formal cardiology, heart failure, advanced practice or consultant-level credentialing.
