The Terros Health Discharge Planners serve as a liaison between the inpatient setting and the assigned clinician at the Outpatient site.
Providers to review presenting clinical information.
The TERROS GMH/SA Discharge Planners serve as a liaison between the inpatient setting and the assigned clinician at the Outpatient site. The Discharge Planner will work with Level I Inpatient Providers to review presenting clinical information, identify resources for successful discharge to the community, and monitor TXIX adult GMH/SA Behavioral Health Recipients (BHRs) follow up with the outpatient provider. The Discharge Planner works under the supervision of the Health Center Director or Site Manager
Duties:
- Discharge Planner activities will be performed in both inpatient and outpatient settings.
- The Discharge Planner will connect individuals receiving Level I inpatient services with ongoing TERROS Outpatient treatment upon discharge.
- The Discharge Planner will work with BHRs while in the hospital, will work with transitioning BHRs, the hospital unit staff, the Outpatient team, family, RBHA care management, PCPs and Health Plans as clinically indicated to ensure effective transition to Outpatient or community services, reduce the risk of readmission to hospital, reduce the utilization of Crisis services and improve the coordination with the PCP and TERROS psychiatric provider.
- Must be able to work well and tactfully with individuals and groups.
- Confidentiality of client records shall be maintained during and after employment at TERROS. Failure to do so may result in dismissal.

- Must possess a combination of behavioral health education and work experience totaling a minimum of four (4) years;
- or a bachelor’s degree in any field, plus one year of work experience in a behavioral health service delivery;
- or a bachelor’s degree in a behavioral health field.
- Computer Skills: Microsoft Word, Excel, Outlook, electronic medical record, NextGen preferred.
- Valid AZ Driver License. Must have valid Arizona driver’s license, be 21 years of age with minimum 4 years driving experience, and meet requirements of Terros Health’s driving policy.
- Must have a valid unrestricted Arizona Fingerprint Clearance Card (Level One) or ability to apply for an Arizona Fingerprint Clearance Card within 7 working days of employment with no disqualifying events.

- Good communication skills, evidenced by a non-judgmental approach, effective listening and positive interactions.
- Must pass a typing test with a minimum speed of 25 WPM.

Skills Required

- Valid unrestricted Arizona driver's license; at least 21 years old, minimum four years of driving experience, and compliance with the employer's driving policy
- Valid unrestricted Arizona Fingerprint Clearance Card Level One, or ability to apply within seven working days without disqualifying events
- Professional interpersonal skills with coworkers, members, families, and community members
- Effective communication, listening, and non-judgmental interaction skills
- Typing speed of at least 25 words per minute
- Successful completion of background check, TB test, and other pre-employment screening
