Aleph Home Care is seeking an experienced and dedicated Director of Nursing to join our team. The Director of Nursing will play a key leadership role in overseeing clinical operations, supporting and guiding our nursing staff, and ensuring that our clients receive the highest quality of safe, compassionate, and personalized care in the comfort of their homes.

Primary Responsibility
Provide the overall direction, management, and supervision of the medical aspects of the Agency.
Specific Responsibilities
• Create and implement policies and procedures. Maintain compliance to ensure Aleph is following state and federal laws and regulations, NJ Division of Consumer Affairs best practices and The Commission on Accreditation for Home Care standards.
• Provide oversight of all Home Health Aides, Companions, and, RN-Supervisors within the home care agency. Oversee the proper education and training of staff hired, as well as annual staff education and evaluation of personnel performance which includes promoting ongoing professional development.
• Oversees the maintenance of medical personnel records and ensure compliance with state and federal laws and accreditation standards.
• Provide staff coaching sessions when needed.
• Oversee and accountable for: initial assessments, reassessments, and phone calls by following up with clients every 30 and 60 days and so on.
• Oversee the maintenance of the client’s clinical records and ensure compliance are followed by the state and federal laws and the accreditation standards.
• Maintain an open line of communication with clients, physicians, pharmacy, and therapists.
• Responsible to create a plan of care and comprehensive assessment for each client and update at least every 6 months or as needed.
• Transitional Care-medication management, oversight of doctor appointments and man- aging the care of clients.
• Complete incident reports as needed and quarterly Performance and Quality Improvement
• Provide 24/7 availability for the scheduling staff based on client needs and emergencies.
• Provide data reports requested by the Home Care Director.
• Assist with additional services and programs that Aleph will provide.
• Assist and support marketing efforts.
• Represent Aleph in varying community committee meetings and events
• Stay informed about changes in the field of nursing and home health care. Continue to review the best practices through the office of Consumer Affairs as well as the CAHC changes.
• Follow Aleph and Jewish Federation policies and procedures.
• In the absence of the Director of Aleph assist in handling any issues or concerns that arise from clients, or staff members.
Qualifications:
• Current license to practice as a Registered Nurse in New Jersey
• BSN with 2 years of professional experience in community health or home care preferred; or RN with 3 years professional experience in community health or home care
• Exceptional customer service and communication skills
• Attention to detail
• Strong management and leadership skills
• Experience with Microsoft Office
