Redwood Coast PACE with Humboldt Senior Resource Center is looking for a full-time Director of Nursing to oversee our nursing services across multiple sites. Redwood Coast PACE — a Program of All-inclusive Care for the Elderly — is a comprehensive health plan for older adults who wish to remain living in their home and community. We provide medical, social, nutritional, rehabilitation, and support services for eligible adults. At Redwood Coast PACE, the goal is to help people stay as healthy and independent as possible. Because their health is monitored on a regular basis, many PACE participants report fewer health crises and better overall health.

JOB SUMMARY: Responsible for oversight of Redwood Coast PACE Nursing Services across the three sites, including management, staffing, and compliance. Directly supervises Registered Nurses (RNs), Licensed Vocational Nurses (LVNs), student interns, and volunteers to facilitate workflows and communication within the team to ensure smooth day-to-day operations across the three sites.


ESSENTIAL DUTIES AND RESONSIBILITIES:

Participant Care and Documentation

  1. Ensures all nursing services are provided to participants daily in accordance with their care plans.
  2. Monitors, oversees, and performs RN assessments.
  3. Monitors general health status of participants daily; consults with Primary Care Provider (PCP) notifying whenever changes in health status are observed.
  4. Performs scheduled and as-needed home visits to assess participant health status, provide skilled nursing care, and reinforce care plans.
  5. Monitors nutritional status of participants and refers to the Registered Dietitian (RD) as required. Works closely with the RD in overseeing daily meals to ensure compliance with dietary restrictions and in following the Care Plan for each participant.
  6. Administers treatments in accordance with physician orders.
  7. Monitors, administers, and records prescribed medications per Title 22 Regulation 54323 and 78317.
  8. Collaborates with Quality to improve participants feeling of safety and satisfaction. 
  9. Responsible for managing medical emergencies in conjunction with Primary Care Providers (PCPs).
  10. Coordinates and participates in on-call coverage in conjunction with PCPs.
  11. Monitors development of Nursing sections of the Care Plans for participants, initially upon enrollment and every six (6) months, specifying short-term and long-term nursing goals. 
  12. Ensures the maintenance of individual participant daily attendance, skilled nursing care records, medication administration records, and other medical data as necessary to document medical and nursing care.
  13. Oversees documentation of the Clinic’s RNs and LVNs.
  14. Facilitates required TB screening and documentation of records for participants.
  15. Completes Incident Reports, as appropriate.
  16. Oversees participant’s clinical needs in the day center.

Administration & Supervision

  1. Collaborates with the Chief Medical Officer (CMO), Director of Redwood Coast PACE, and Director of Human Resources on the employee recruitment and disciplinary processes.
  2. Completes Interdisciplinary Team discipline-specific competencies for direct reports.
  3. Ensures and maintains adequate daily staffing across all three clinic sites.
  4. Ensures ongoing and program-compliant in-service training and documentation for employees, contractors, and volunteers.
  5. Ensures regulatory compliance, including CMS and DHCS expectations.
  6. Reports to the CMO and Director of Redwood Coast PACE concerning personnel matters of the Clinic, including situations or occurrences that impact employee relations and morale.
  7. Participates in program and department policy and procedure development.
  8. Enforces written agency and program policies.
  9. Works closely with department leads to identify areas for process efficiency and implement improvements. Support system improvements by contributing to the development and refinement of procedures that optimize workflow.
  10. Ensures alignment of team efforts with organizational goals, promoting a collaborative and efficient working environment.
  11. Monitors and analyzes team performance to identify opportunities for continuous improvement.
  12. Coordinates training for Care Partners regarding nursing services, as requested.
  13. Monitors the safety of the Nursing work environment and enforces safety practices with employees.
  14. Develops and maintains systems for quality health delivery. Ability to run reports and assess data to identify areas of improvement.
  15. Ensures best practices are implemented via ongoing training for staff.
  16. Provides team building and support to create a professional collaborative working environment.
  17. Facilitates daily huddle with nursing staff to make sure tasks are covered.
  18. Demonstrates behaviors consistent with agency values.
  19. Additional duties may be assigned, as needed, that align with the responsibilities of the position and support the vision, mission, and values of the organization.

Coordination

  1. Attends and participates in all required leadership meetings and trainings. Coordinates Clinic in-services with other departments.
  2. Coordinates and collaborates with other department leads to enhance interdepartmental communication and teamwork.
  3. Helps coordinate delivery of all nursing topics in-service trainings. Ensures educational materials and records of attendance are maintained.
  4. Develops and updates clinic and nursing policy and procedures.
  5. Coordinates with Care Kinesis regarding medication management issues.

Advocacy and Participant/Family Education

  1. Attends meetings with outside agencies, caregivers, and families. Networks with other agencies on behalf of the program and participants.
  2. Provides training to participants and families regarding medications, medical conditions, safety, etc.
  3. Communicates with physicians and caregivers of participants regarding participant health concerns and makes recommendations.
Qualifications

EDUCATION &/or EXPERIENCE:

  1. Graduate of an accredited school of professional nursing required.
  2. Minimum three (3) years of nursing experience required; five (5) years or more preferred.
  3. Minimum one (1) year working with a frail or elderly population required.
  4. Minimum two (2) years of experience working in a supervisory or leadership position required.
  5. Minimum one (1) year experience working in a hospital is preferred.

 

LICENSES OR CERTIFICATES:

  1. Must be a current RN licensed by the California Board of Registered Nursing, in good standing.
  2. Current CPR and First Aid certification.
  3. Valid California Driver’s License, and personal auto liability insurance required. Must have a personal vehicle in good driving/operating condition and insured for State minimum liability requirements that can be used for the applicable job functions noted above.

 

PRE-SCREENING REQUIREMENTS:

  1. Requires clearance of a DOJ and FBI criminal history background check
  2. Pre-employment physical and PPD skin test.
  3. Requires Motor Vehicle Report verification.
  4. COVID-19 and Influenza (flu) vaccines preferred.

SUPERVISORY REQUIREMENTS:

  1. This position supervises the following employees: Registered Nurses (RN), Licensed Vocational Nurses (LVN), and Medication and Discharge Nurse.
  2. In coordination with Human Resources and other supervisory staff, overseas hiring, orienting, training, evaluating, disciplining, and terminating of specified departmental staff.

 

PHYSICAL REQUIREMENTS:

This position requires the ability to perform a combination of sedentary and physically active duties in both clinical and community-based settings. The LVN must be able to sit, stand, and walk for extended periods; frequently bend, stoop, reach, and twist; and use hands for fine motor tasks such as medication administration, documentation, and operating medical equipment. The role requires the ability to lift, carry, push, and pull up to 50 pounds regularly, and occasionally assist with heavier transfers (with appropriate equipment or assistance), including supporting participants with ambulation, transfers, and wheelchair mobility. The position also requires visual acuity and manual dexterity sufficient to perform clinical tasks, as well as the ability to travel between locations, including driving, and safely navigate a variety of environments such as participant homes and community settings. Also requires the ability to operate a motor vehicle safely and travel to participant homes.
