The CSAC Case Manager is responsible for assessment, service plan development, short-term counseling and referrals for guests, with the emphasis of working with guests who are dealing with substance use or substance abuse. Ensures that client input is included in every step of the process. This position provides quality case management services that connects the client with appropriate benefits, assistance and support with the goal of stabilization and connection to a provision of safe, decent and affordable housing.

Compensation:

Starting from $24 hourly, actual pay commensurate with experience within contract budgets.

Essential Functions:

  1. Demonstrate excellent customer service consistent with the standards contained in the Vision, Mission, and Values of the organization.
  2. Manage a caseload and ensure positive outcomes through the timely coordination of all case management services with a warm and welcoming attitude; Meet contract standards and provide linkages to substance abuse or mental health treatment as needed, and transportation to other involved or needed providers to coordinate member’s care and housing solutions.
  3. Provide supportive counseling and participate in crisis intervention services with individuals including mental status and risk assessment and linkage to crisis teams and hospitals; coordinate written crisis plans when indicated in coordination with the client's psychiatric provider.
  4. Timely and accurately document clinical records in the required systems of record and maintain records of referrals, assessments and placement plans in compliance with IHS standards of documentation and all contract requirements.
  5. Make collaborative outreach attempts, including transportation of clients to other involved or needed providers to coordinate member’s care as necessary.

Required Knowledge/Skills/Abilities:

  1. Knowledge of homelessness and the issue associated with it; working knowledge of mental health issues and advanced knowledge in the treatment of substance abuse issues.
  2. Strong written communication and interpersonal skills with an emphasis on conflict resolution.
  3. Excellent time management skills with ability to manage a high volume of cases.
  4. Possess personal qualities of integrity, patience and commitment to mission.
  5. Flexible and able to multitask; can work within an ambiguous, fast-moving environment.

Required Education and Experience:

  1. High School diploma or GED equivalent (Bachelor's preferred).
  2. Current CSAC certification.
  3. Two years of direct case management or supportive services experience working with homeless or other disenfranchised individuals.
  4. Valid Driver’s License and clean traffic abstract.

Position Type/Expected Hours of Work:

Regular Full-Time classification. General business hours, Monday through Friday, including some evenings, weekends and holidays. Hours and days may vary based on the needs of the organization.
