Greater Nashua Mental Health is seeking a Community Support Specialist to provide community-based support to individuals working toward recovery from mental health and co-occurring conditions. This role focuses on building engagement, supporting independence, and helping clients improve functioning within their natural environments through individualized, strengths-based interventions.
Key Responsibilities
- Provide direct, community-based support services to individuals in their homes and community settings.
- Engage clients using evidence-based interventions such as Motivational Interviewing.
- Maintain a caseload and meet productivity expectations for direct client service hours.
- Conduct outreach to re-engage clients who have disengaged from treatment.
- Complete accurate and timely documentation for all services and outreach activities.
- Collaborate with multidisciplinary teams to support coordinated, client-centered care.
- Participate in team meetings, supervision, and ongoing training activities.
- Maintain schedules and document services in the Electronic Medical Record (EMR).
- Support clients in building life skills, accessing resources, and achieving recovery goals.
- Complete mileage and expense reporting in a timely manner.
- Perform additional duties or special projects as assigned.
Qualifications
- Associate’s degree in Psychology or related field required; Bachelor’s degree preferred.
- 2+ years of experience in human services or behavioral health field.
- Knowledge of mental health conditions and recovery-oriented practices.
- Strong communication, interpersonal, and engagement skills.
- Ability to work independently and collaboratively within a team environment.
- Strong organizational and time management skills.
- Proficiency with Microsoft Office and electronic documentation systems.
- Ability to exercise sound judgment and maintain professional boundaries.

Salary Range: $19.35/hr – $25.59/hr

Skills Required
