Job Description Summary:

The Community Resources Coordinator/Priority Populations Coordinator manages supportive services, resource coordination, education and benefits navigation for priority populations within the community. Specifically, this position will provide information and referral services to individuals under the age of 60 who are living with a disability. In addition, this position is responsible for implementing the agency’s National Family Caregiver Support Program including education/outreach, screening, referral and coordination of services as well as documentation and reporting. This role works within the Wellness Team at the Office on Aging. Job Description:

Budgeted Salary: $54,080 annually

Essential Duties:

  • Program Coordination: Oversee daily operations, participant intake, and service delivery for caregiver support initiatives.
  • Planning, Outreach & Education: Plan and implement community outreach events, training opportunities and educational programming for family caregivers and care partners.
  • Resource Referrals: Connect individuals and families to respite care, supplemental services, and community benefits. In addition to working with caregivers/families, incumbent will provider resources, information and assistance to those under 60 years of age who are living with disabilities.
  • Internal Coordination: Work with internal staff to receive referrals and coordinate care as well as working with fiscal staff to ensure availability of funds for requested services.
  • Compliance & Reporting: Track service utilization, maintain accurate client records, and meet grant reporting rules. 
  • Performs related administrative duties, such as maintaining case records, and preparing statistical data reports using required software as required by the State.

Years of Experience:
2 years of experience in case management, aging services, caregiving support or service coordination for disabled populations, or related experience
Education:
Bachelor's Degree in Social Work, Human Services, or a related field
Knowledge, Skills, and Abilities:

  • Knowledge of aging programs, disability benefits, and community resources
  • Ability to maintain composure, even in very difficult situations
  • Ability to manage conflict and tactfully handle complaints and difficult situations
  • Knowledge of crises Intervention theory
  • Knowledge of administrative and clerical procedures
  • Knowledge of terminology used in diagnosis and classification of illnesses, injuries and disabilities as well as causes and consequences of illness, injuries and disabilities
  • Knowledge of community resources and services for client population
  • Knowledge of documentation procedures
  • Ability to communicate effectively, both verbally and in writing, adjusts communication style or approach depending on the audience
  • Ability to manage conflict and tactfully handle complaints and difficult situations
  • Skilled listener, ability to practice actively listening
  • Ability to conceptualize, plan and manage multiple projects or tasks
  • Ability to remain calm in a crisis

COMPENSATION:
Other compensation may include but is not limited to:
