We are fully remote with team members nationwide and will stay that way.  We value mental health: the willingness to be vulnerable is a requirement to join Daybreak.   We have regular mental health days (at least once per quarter!).  Lastly, we value fun and hold regular virtual and in-person activities.

The Role

Daybreak seeks a Community Outreach Liaison to help build community awareness around the therapeutic program, expand use of Daybreak’s referral system, and increase student access. As the Community Outreach Liaison, you will serve as Daybreak’s “boots on the ground”, leading school-based presentations, building relationships with referring parties, and helping to promote therapeutic care across the community.

The ideal candidate for this role has worked deeply in the Los Angeles community and is passionate about advancing youth mental health across the community. This person is comfortable working with a variety of stakeholders and is loves to work with a considerable amount of autonomy in organizing and prioritizing outreach. This person enjoys being on the road much of the week and building new relationships 

Community Outreach - 75%

  • Visit with designated schools across the district to help develop, sustain and grow relationships with (but not limited to) site administrators, social workers, wellness coaches, counselors, educators, parent liaisons, and more.
  • Help school staff, community partners, families, and students understand the value of Daybreak’s program and be able to easily refer students to care. 
  • Coordinate efforts to grow adoption of Daybreak among schools in the district, including tracking and ensuring the efficacy of various outreach activities. Outreach activities could include educational presentations, meeting 1:1 with referring parties, leading information sessions, participating in wellness fairs, facilitating universal screeners, attending student assemblies, and more.
  • Support families with onboarding and session attendance by providing 1:1 support and “train the trainer” sessions for care navigators.
  • Provide initial case consultations to patients and their families through video technology, telephone calls, and in-person visits
  • Establishes and maintains relationships with community agencies with the goal of minimizing waitlists and providing interim services
  • Meet or exceed established enrollment targets

Planning & Documentation - 15%

  • Completes documentation within 48 hours
  • Consistent planning, tracking and documenting of activities
  • Weekly meetings with site-based teams, enrollment leads, and others

Project Management - 10%

  • Coordination with schools across the district to align resources and educational opportunities
  • Create 6 to 12 month districtwide plans in collaboration with account management and marketing

Qualifications

  • Lives in Wake County, North Carolina and willing to visit school sites across the district, traveling to school sites up to 80% of the week
  • Proven track record of building referral base and relationships, such as business development and/or sales, community outreach or referral management for a hospital system or payer
  • Current driver’s license, proof of acceptable automobile insurance coverage, and reliable transportation
  • Excellent oral and written communication skills required including well-developed public speaking skills
  • Thrives in a fast-paced, technical, and mission-focused environment
  • Must have general computer skills and a working knowledge of the internet and the ability to independently learn and use Google Suite and Salesforce
  • Ability to take initiative and work autonomously
  • Demonstrates high ethical standards of behavior
  • Willingness to work flexible hours, including evenings and weekends as required
  • Previous knowledge of, and passion for youth Mental Health

Nice to Have 

  • Experience in case management or behavioral health
  • Bilingual in English and Spanish is highly preferred 
  • SWA, LCSW-A or pursuing education in social work 
  • Former educators or school administrators are encouraged to apply

Work Environment

This position will require up to four days per week working in the community. The remainder of the work week will be a remotely based home office environment. 


Position Type and Expected Hours of Work

This position is 40 hours a week. Team members may work with their managers to adjust work hours to suit the needs of the position. This position requires time at events that fall outside of normal working hours and team members will be asked to work additional days as work demands require, including weekends. 


Travel

Regional travel (outside of local area) may be required up to 10% of the time.
