Job Summary
We are seeking a motivated and compassionate Community Liaison to promote our skilled homecare services, build relationships with healthcare professionals and community organizations, and help connect individuals and families with quality homecare. The ideal candidate is a strong communicator with experience in home healthcare marketing, sales or community outreach. 

Key Responsibilities

  • Develops and maintain referral relationships with hospitals, physicians, rehab/SNF centers, assisted living communities, senior centers and other healthcare providers
  • Promotes the agency's homecare services through community outreach, presentations, networking events and health fairs
  • Identify new referral opportunities and implement strategies to increase client referrals
  • Maintain accurate records of referral activities and marketing efforts
  • Collaborate with the team to ensure smooth client admission and exceptional customer service
  • Represent the agency professionally while uploading privacy and regulatory requirements

Qualifications

  • Experience in home healthcare marketing, hospice, or community outreach is preferred
  • Strong networking, relationship building and communication skills
  • Self motivated with the ability to work independently and meet referral goals
  • Valid driver's license
  • Proficiency in basic computer software
