The Clinical Partnership Coordinator is a field-based position that combines clinical support with facility relationship management to help deliver exceptional specialty care throughout Skilled Nursing and Long-Term Care communities. Reporting to Business Development and working in partnership with Clinical Operations, this role serves as the face of Alteas Health within assigned facilities. The Clinical Partnership Coordinator supports the success of Alteas Health's PRIME and Principal Care Management (PCM) programs by conducting spirometry testing, coordinating eligible patient enrollments, maintaining strong facility partnerships, and ensuring operational excellence through consistent communication and follow-through.

This position is ideal for a healthcare professional who enjoys working independently, building lasting relationships, and making a meaningful impact on both patient outcomes and client satisfaction throughout St. Louis and Southern Illinois while periodically supporting the Kansas City, Missouri and Kansas market.

What You'll Do

  • Conduct spirometry testing in accordance with company protocols, provider orders, and quality standards.
  • Prepare patients for spirometry testing while ensuring accurate testing conditions and quality results.
  • Support the success of Alteas Health's PRIME program through routine clinical program coordination and facility engagement.
  • Identify eligible patients and coordinate Principal Care Management (PCM) enrollments.
  • Educate patients and facility staff regarding PRIME and PCM services and program expectations.
  • Coordinate PRIME clinic schedules with providers and facility staff to maximize participation and operational efficiency.
  • Maintain accurate documentation of spirometry testing, PCM enrollments, facility visits, and clinical program activities.
  • Serve as the primary operational representative of Alteas Health within assigned partner facilities.
  • Develop strong working relationships with Administrators, Directors of Nursing, providers, Social Services, Therapy departments, and facility leadership.
  • Participate in QAPI meetings, implementation activities, and other client-facing operational meetings.
  • Track operational concerns, communicate updates, and ensure timely issue resolution through completion.
  • Collaborate with Business Development, Clinical Operations, providers, and internal departments to deliver an exceptional client experience.
  • Support additional organizational initiatives and special projects as assigned.

What We're Looking For

Required Qualifications

  • High School Diploma or equivalent.
  • Current Certified Medical Assistant (CMA), Registered Medical Assistant (RMA), or Medical Assistant (MA) certification.
  • Current BLS certification or ability to obtain within company-established timeframes.
  • Minimum two (2) years of Medical Assistant, clinical support, or comparable healthcare experience.
  • • Valid driver's license, reliable transportation, and the ability to travel daily throughout St. Louis and Southern Illinois, with routine monthly travel to the Kansas City, Missouri and Kansas market.
  • Strong organizational, communication, and relationship-building skills.

Preferred Qualifications

  • Spirometry testing experience.
  • Pulmonology, Cardiology, or Internal Medicine experience.
  • Experience with Principal Care Management (PCM), Chronic Care Management (CCM), or other value-based care programs.
  • Experience participating in QAPI or quality improvement initiatives

Compensation

Compensation is determined based on experience, education, certifications, demonstrated competencies, geographic location, market conditions, organizational needs, internal equity, and other relevant business considerations.

Work Environment & Travel

This position is 75-100% field-based and requires daily travel throughout St. Louis, MO and Southern Illinois. The majority of each workweek is spent visiting partner facilities to conduct spirometry testing, support PRIME and PCM initiatives, participate in operational meetings, maintain facility relationships, and provide ongoing client support. Monthly travel to Kansas City (Kansas & Missouri) will required to support partner facilities. Overnight travel may be required based on business needs.
