This position is accountable to the Program Director of the inpatient rehabilitation program. The main job duties are: 1.) to respond to, and carry
out, pre-admission screenings for patients referred to the rehabilitation program, 2.). to assist in entering referral and admission data into a
database, 3) compile monthly and quarterly trend reports and analyzes the data with recommendations for improvement or continued success and
4) to provide education regarding inpatient rehabilitation to a variety of stakeholders, 5) has the overall responsibility of evaluating and consulting
with PM&R physician in determining if patients meet criteria for admission to Acute Inpatient Rehab
- Responsible for the assessment of patients referred for inpatient rehabilitation services; makes
recommendations for alternate levels of care within the continuum to referral sources if the patient is not a
candidate for admission . These assessments wll be internal or external and may require travel to hospitals,
physician offices, patient residence or emergency rooms. Will enter all required referral and admission data into
the provided computer evaluation tool and database - Coordinates the gathering of pertinent patient information on referred patients from various sources, including
the medical record, onsite and telephonic interviews, and through other electronic media . Consults with the
Admissions Coordinator to enable referral to advance through admissions process. - Communicates quickly and effectively with all referral sources at every stage of the referral process as a means
to achieve referral source satisfaction and to advocate for patients by providing a prompt decision regarding
admission . - Initiates the flow of patient information to the Program Director and Medical Director for review and decisionmaking.
- Initiates the admission of patients by fully completing all necessary forms and insurance pre-certifications.
Seeks out pre-certifications when appropriate to patient needs. - Other related job tasks or responsibilities as assigned
- Special Skills : Ability to comprehensively review a medical record for inpatient rehabilitation medical necessity; routinely demonstrates good
organizational skills; is results-oriented and independently goal-directed; is able to multi-task and able to communicate well with many diverse
audiences - professional and non-professional; is able to meet established timeframes; demonstrates good customer service skills; is able to work
independently; is able to pass the FIM competency and pre-admissions screening competency examinations; has experience with computers and
other electronic data entry devices; must have valid driver’s license. Travel with own vehicle required, eligible for mileage reimbursement with
appropriate tracking submitted. Must be able and willing to travel via own personal vehicle for in town in the Phoenix Metropolitan Area at least 50%

- Experience: Must have a minimum of 2 years of clinical experience, with clinical expertise and strong interpersonal skills having been
demonstrated; experience in rehabilitation setting preferred. - Education: Graduate from an accredited nursing program as an RN or has other clinical licensure as a PT, OT or SLP
- Certifications: Registered Nurse Licensure or Clinical licensure required applicable to the provision of inpatient
rehabilitation services; PT, OT, SLP

Skills Required
- Minimum 2 years of clinical experience
