Overview

In collaboration with patients, families (as defined by the patient) and staff across all disciplines and departments is responsible for planning, coordinating and evaluating nutrition and dietary services and programs. 


Hybrid work schedule with the ability to work remotely one day per week, based on business and operational needs.


Salary range for this position will start at $66,000.00/Yr

Responsibilities

Essential Duties:• Evaluate the dietary needs of patients, develop and implement a dietary plan, and document nutritional information in patient medical charts.• Educate patients on food selection to maintain and promote health as well as to prevent and treat illness and disease.• Develop and conduct specialized patient nutrition classes and individual patient instruction.• Provide information and education to staff members using nutrition education materials or in-service/meetings per request.• Implement and monitor quality control programs.• Evaluate programs for effectiveness and accomplishment of goals.• May be responsible for educational instruction or instructional team participation for the DMST program.• May participate in or manage special projects/programs as needed.• Perform other duties as assigned.

Primary Accountabilities:• Ensure all tasks provided and associated with patient care, patient administrative processes and related duties comply with all regulatory and accreditation standards including The Joint Commission and CommUnityCare Standard Operating Procedures and CommUnityCare Policies and Procedures.• Develop and maintain favorable internal relationships, partnerships with co-workers, including clinical managers, clinical support staff, providers and business office staff.• Interact respectfully and collaboratively with patients and their families, striving to develop favorable relationships with families.• Collaborate with all members of the care team in providing patient-centered care.• Ensure all actions, job performance, personal conduct and communications represent CommUnityCare in a highly professional manner at all times.• Uphold and ensure compliance and attention to all company policies and procedures as well as the overall mission and values of the organization.• Complete all mandatory training and maintain appropriate credentials/licensure.

Knowledge/Skills/Abilities:• High level of skill at building relationships and providing excellent customer service.• Basic computer systems knowledge, including but not limited to Microsoft Office products.• Strong attention to detail and accuracy.• Excellent verbal and written communication skills.• Ability to apply interpretations of federal, state and local regulations.• Ability to examine and evaluate the need for nutrition and dietary services and programs.• Bilingual (English/Spanish) desirable.

Qualifications

Education:

Bachelor’s degree in Dietetics. -Required


Experience: • Experience providing nutritional services in an inpatient/outpatient, one-on-one or group education setting. - Required• Demonstrated knowledge of foods, nutrition and food systems management.  - Required• Extensive knowledge of available resources in the field of nutrition education and dietary services. - Required• Experience developing and conducting on-going in-service training programs. - Required• Experience developing and conducting specialized nutrition classes/individual patient instructions. - Required• Experience in program/project management. - Preferred


Licenses/Certifications:

Current license in the state of Texas as a Registered Dietitian (RD). - Required (Upon Hire)

Current Health Care Provider Cardiopulmonary Resuscitation (CPR) certification through American Heart Association or American Red Cross. Will be required to maintain a current CPR certification during employment. - Required (Upon Hire)
