Position Overview:

As a Client Success Coordinator at Forge Health, you will be a vital part of our team, dedicated to ensuring an exceptional experience for both our clients and referral partners. Your primary focus will be to facilitate client transitions into care, manage administrative tasks, and support high-acuity clients through proactive outreach. This role requires a blend of strong administrative skills, compassionate client interaction, and effective communication with referral sources.
Please note, this position is fully remote, Monday-Friday.


Key Responsibilities:

  • Handle incoming calls to the Intake Line, directing prospective clients to the appropriate Forge Health office to complete their admission process.
  • Manage essential documentation, including client information entry, clinical records, insurance cards, ID cards, and Release of Information (ROI) forms, to support efficient referral processing.
  • Record new client inquiries, provide updates on client status, and support documentation needs for partnerships, including client outcome reporting and follow-up.
  • Provide intake support and follow-up outreach to clients referred to our Intensive Outpatient Program (IOP), ensuring their needs are addressed and guiding them through required steps.
  • Collaborate with clinical and administrative teams to manage urgent needs or concerns related to high-acuity cases.
  • Streamline administrative workflows to enhance efficiency in the admissions process and ensure an exceptional experience for all stakeholders.
  • Work with internal teams to identify areas for process improvement and implement best practices.
  • Manage client records, including data entry and updates, while maintaining confidentiality in accordance with HIPAA regulations.
  • Schedule appointments, follow up on cancellations, and coordinate with the front office to manage client flow.
  • Coordinate with external providers and community resources as needed to support client care plans.
  • Participate in quality assurance initiatives to enhance client satisfaction and outcomes.


Qualifications:

  • Bachelor’s degree in psychology, Social Work, Healthcare Administration, or a related field, preferred.
  • Minimum of three (3) years of job-related experience providing mental health or substance abuse assessments and referrals.
  • Proficient in Microsoft Office Suite and experience with electronic health records (EHR) systems.
  • Strong understanding of the Behavioral Health System and levels of care
  • Knowledge of substance use disorders and mental health conditions
  • Excellent interpersonal, organizational, writing, and computer skills
  • Extremely detail-oriented with excellent analytical, problem-solving, and organizational skills
  • Familiarity with HIPAA regulations and confidentiality requirements.
  • Works well in a remote environment, with minimal supervision.
  • Flexible, adaptive, punctual, and highly reliable.
