As a Case Manager, you'll be a vital member of our patient-centered care team, ensuring seamless coordination and advocacy for patients and their families. QualificationsWhat You Will Need
Required Qualifications
- Bachelor¿s degree in nursing or Master¿s degree in Social Work.
- Minimum 3 years of clinical experience.
- Valid, unrestricted State of Michigan license as a Registered Nurse (RN) or Licensed Social Worker (LMSW).
- Excellent verbal communication, written documentation, and interpersonal skills.
- Strong problem-solving, analytical, and decision-making abilities.
- Proficiency in computer skills and knowledge of healthcare systems.
Preferred Qualifications
- Certification in Case Management (CCM) or Accredited Case Manager (ACM).
- Experience in discharge planning, home health care, rehabilitative medicine, or managed care.
