Family & Children’s Center is seeking a compassionate, motivated Case Manager to join our Community Support Program (CSP).

In this role, you’ll work directly with individuals living with severe and persistent mental illness, helping them navigate challenges, connect with resources, develop support systems, manage symptoms, and work toward their personal recovery goals.

What You’ll Do

As a CSP Case Manager, you will:

  • Provide mental health case management services to individuals with severe and persistent mental illness.
  • Complete assessments and create treatment, recovery, and crisis plans.
  • Help clients identify and achieve their individual recovery goals.
  • Provide strengths-based, recovery-oriented services.
  • Help clients manage psychiatric symptoms and co-occurring medical and substance use concerns.
  • Provide symptom monitoring, education, crisis intervention, and medication education/monitoring as directed.
  • Connect clients with community resources related to housing, food, healthcare, safety, and independent living.
  • Coordinate care with internal and external treatment providers.
  • Engage family members and support systems when permitted by the client.
  • Participate in treatment meetings and ongoing supervision.
  • Maintain accurate, timely clinical and case management documentation.
  • Provide after-hours crisis services by phone or face-to-face when needed.
  • Assist with discharge planning and referrals.

This is a community-based position, with at least 50% of the work occurring in community settings or clients' homes. Local travel is required.

Required Qualifications

Education & Experience:

  • Bachelor's degree in behavioral science or a related field plus 1,000 hours of supervised post-degree clinical experience with persons with chronic mental illness; OR
  • Bachelor's degree in another field plus 2,000 hours of supervised post-degree clinical experience with persons with chronic mental illness.

Candidates should also have familiarity with the human service delivery system, DHS 63, recovery principles, ethics, and professional boundaries.

A valid driver's license and ability to use your own vehicle for client transportation are required.

Preferred Qualifications

  • Master's degree in Social Work, Clinical Psychology, or Psychiatric Mental Health Nursing
  • Additional supervised clinical experience working with individuals with severe and persistent mental illness
  • Previous Community Support Program (CSP) experience

Skills Required

  • Bachelor’s degree in behavioral science or a related field with 1,000 hours of supervised post-degree clinical experience with people with chronic or severe and persistent mental illness
  • Alternatively, bachelor’s degree in a non-behavioral-science field with 2,000 hours of supervised post-degree clinical experience with people with chronic or severe and persistent mental illness
  • Familiarity with the human service delivery system, DHS 63, recovery principles, ethics, and professional boundaries
  • Ability to use Microsoft Office, email, scanner, fax machine, and enterprise software
  • Valid driver’s license and ability to use a personal vehicle for client transportation
  • Master’s degree in social work, clinical psychology, or psychiatric mental health nursing
  • Equivalent education with 3,000 hours of supervised clinical experience outside a CSP program or 1,500 hours within a CSP program
