Pay rate: $21.00-$24.00/hour

The Case Manager is responsible for:

  • Providing documented individual and group clinical services and case management to clients with mental health, substance abuse, or dual diagnosis problems.
  • Developing implements and revising individual treatment plans for assigned clients in established time frames with the participation of the client.
  • Referring clients to appropriate services in the community as necessary.
  • Participating in multidisciplinary team treatment planning, case review and case conferences.
Qualifications

Qualifications:

  • LADC I or CADAC certification preferred but not required. 
  • Minimum of a high school diploma or equivalent 
  • At least one year experience working in a substance abuse treatment, case management role or closely related field. 
  • Strong documentation, organizational, and communication skills
  • Must be able to express self adequately in written and oral communication and to communicate effectively in an interdisciplinary, culturally diverse setting with patients, families, staff and regulatory agencies 
  • Must have basic ability in use of computers to be able make entries and utilize electronic health records 

Skills Required

  • Minimum high school diploma or equivalent
  • At least one year of experience in substance abuse treatment, case management, or a closely related field
  • Strong documentation, organizational, and communication skills
  • Ability to communicate effectively in written and oral formats with patients, families, staff, and regulatory agencies
  • Basic computer skills, including making entries and using electronic health records
  • LADC I or CADAC certification
