MAJOR FUNCTIONS:

An Office of Mental Health (OMH) Housing Case Manager plays a crucial role in assisting individuals with mental health challenges to secure and maintain stable housing. The primary responsibilities include conducting assessments, developing personalized service plans, connecting clients with necessary services, and ensuring compliance with housing regulations. The role requires a combination of direct client support, coordination with service providers, and administrative tasks to promote clients' independence and well-being.

SPECIFIC DUTIES & RESPONSIBILITIES:

  1. Client Assessment and Service Planning: Conduct comprehensive assessments to identify clients' housing and support needs. Develop individualized service plans with short-term and long-term goals, regularly reviewing and updating them to ensure they meet clients' evolving needs. 
  2. Service Coordination: Connect clients to essential services, including medical care, mental health support, substance use treatment, legal assistance, and entitlement programs. Act as a liaison between clients and external organizations to advocate for their needs. 
  3. Direct Support and Monitoring: Provide ongoing support through regular home visits (at least one per month) and telephone contacts. Monitor clients' progress toward their goals, document interactions, and adjust service plans as necessary. 
  4. Crisis Intervention: Respond to crisis situations promptly, employing de-escalation techniques and coordinating with appropriate services to ensure clients' safety and stability. 
  5. Documentation and Compliance: Maintain accurate and up-to-date case records, including service plans, progress notes, and other required documentation. Ensure all activities comply with OMH guidelines and regulations, preparing for audits as necessary. 
  6. Client Empowerment: Assist clients in developing life skills necessary for independent living, such as budgeting, cooking, and community navigation. Provide education on tenants' rights and responsibilities to promote self-sufficiency

REQUIREMENTS: EDUCATION AND EXPERIENCE KNOWLEDGE, SKILLS AND ABILITIES: 

  • A bachelor's degree in social work, psychology, or a related field is typically required. 
  • At least one to two years of experience in case management or working with individuals with mental health disorders. 
  • Experience in housing assistance and knowledge of OMH programs are preferred.
