General Description:

The Case Manager is primarily responsible for providing support for patients with chronic or acute illness, providing patient education and making referrals for other services. The Case Manager may rotate on occasion or as necessary, to other DRCHC sites. (Bossier, Gilliam, Haynesville, Minden, or Shreveport).

Work Experience Requirements:

  • Receive inbound and outbound calls to/from patients regarding clinical care and services and healthcare related needs regarding scheduling appointments, follow-ups, referrals, etc. 
  • Serve as an Administrative Assistant in generating/completing memos, reports, forms, maintain providers’ calendars, ordering supplies and maintenance of the filing system, etc. 
  • Ensures all confidential personal health information and financial information is entered into computerized system with a high rate of accuracy. 
  • Maintain accurate, timely and secure documentation in compliance with regulatory standards.
  • Complies with all federal and local laws in ensuring patient privacy and DRCHC policies and procedures.
  • Performs additional duties as necessary.

Education/Experience Requirements: (The minimum qualifications can be with any combination of education and/or experience.)

  • High School Diploma or equivalent 
  • Associate’s degree in business or healthcare administration or related preferred.
  • At least three years of patient/customer services experience.
  • Strong communication and interpersonal skills.
  • Experience in behavioral health preferred.

Licensure/Certifications:

  • BLS (American Heart Association)

Skills Required

  • High school diploma or equivalent
  • Associate's degree in business, healthcare administration, or a related field
  • At least three years of patient or customer service experience
  • Strong communication and interpersonal skills
  • Behavioral health experience
  • Basic Life Support certification from the American Heart Association
