HeartShare St. Vincent’s Services (HSVS) Nuevo Camino program is dedicated to providing 38 unaccompanied minors, ages 5-12, with a supportive, nurturing environment through transitional foster homes, daily educational services Monday - Friday, and supportive wrap-around services in the Sunset Park neighborhodd section of New York City. Our program is rooted in ensuring that commited to ensuring that service delivery is trauma informed, culturally appropriate, and tailored to the needs of each child while complying with ORR (Office of Refugee Resettlements) program maintaining all program requirements. of ORR. 

Nuevo Camino is looking seeking for a case manager to provide oversight and support to these young persons children. Major responsibilities of the Caseworker are to  identify an appropriate sponsor, either parents or guardians to whom the young personschild may be released unified with. The Caseworker assess the safety and well being and, and to  elevates identify any concerns found found with the potential sponsors and discusses with the Nuevo Camino team and Federal Field Specialist. The Caseworker is also responsible for ensuring that the young personchild in the transitional home is receiving essential services such as: medical and dental care, mental health;, and dental care; basic education; orientation; and legal screening. Due to program requirements.

Primary Responsibilities and Essential Functions of Position:

  • Strong communication and writing skills.
  • Ability to work collaboratively with clients and co-workers.
  • Knowledgeable about working with immigrant youth population.
  • Superior organizational and time management skills. and an ability to work independently and as part of a team. Ability to manage time effectively, multi-task, and determine priorities.
  • An ability to work independently and as part of a team.
  • Knowledgeable about ACS/OCFS regulations governing disciplinary actions.
  • Experience and facility working with Microsoft Office Applications.
  • Ability to manage time effectively, multi-task, and determine priorities.
  • Ability to work flexible hours as needed.
  • CPR-Certified a plus.
  • HSVS requires all new hires to provide proof of vaccination prior to their start date. Employees who require an accommodation for medical or religious reasons should notify Human Resources.
Qualifications
  • Bachelor's degree from four year college or equivalent​ required
  • 1 year of similar experience required
  • Bilingual and Fluent in English and Spanish required
