Position Summary/Objective
The Case Manager promotes a culture of safety, hospitality, service, honor, and respect among our residents, our staff, and volunteers, for the purpose of furthering the vision and mission of the St. Vincent de Paul Society. Working under general direction of the Program Manger the Case Manager, coordinates and provides social services to women and families within the Kettering Commons program.
This Case Managers primary responsibility is to work with individuals to ensure that developmental needs are being met, work with the families on supporting and developing parental skills, work with the youth on developing skills necessary for general wellness and coordinates with the schools and family system to ensure that academic needs and supports are in place so that the youth receive the most appropriate intervention and supports.
Essential functions
- Assist clients in obtaining and maintaining necessary social service supports and linkage to outside providers. Assist clients in linking to employment programming and/or other income such as linkage to Social Security Administration. Assist clients in obtaining life skills necessary to live independently while utilizing a person-centered approach.
- Regularly assess psychological status of clients.
- Develop goals for individuals within the program that focuses on the uplifting positive growth, linkage to social support, etc. and update every six months.
- Communicate with physicians to report patient progress, needs, concerns, and potential and actual problems.
- Develop and maintain thorough knowledge of referral resources throughout the community.
- Maintain up-to-date knowledge of Medicare, Medicaid, and other third party reimbursement sources. Gain and maintain knowledge of CARF and OHMAS accreditation systems.
- Develop working relationships with leaders in community resource groups; represent the agency at meetings and on various committees that address the social needs of clients.
- Maintain accurate and timely records, including clinical and progress notes.
- Maintain up-to-date knowledge of the professional social work field by participating in continuing education activities.
- Provide positive client relations when interacting with clients, families, physicians, fellow department employees, referral sources, service agencies, and all other contacts.
- Participate in the ongoing assessment process and develop interventions to support clients, individually, within their family, and in a group setting.
- Act in compliance with all agency policies and procedures including client rights, safety rules and regulations, the agency code of ethics, and all applicable state and local rules.
- Prepare and maintain all required client documentation in a timely manner.
- Utilize knowledge of community resources to establish linkages and to serve as a liaison between St. Vincent de Paul and other agencies and community groups to ensure that the needs of the clients are being addressed.
- Assists in being on-call one week out of each month, after hours, during which time resident/facility needs, that are screened as emergent, are addressed.
- Assists families served in creating housing readiness plans and supports families in meeting independent housing goals identified as a positive fit for individuals and families.
Physical demands
Must be able to lift up to approximately 50 lbs during client moves, Food Bank day, etc.
Travel percentage
- The position travels occasionally on company and/or personal time.

QualificationsRequired education and experience
- High School diploma or equivalent
- Experience working with clients diagnosed with substance use disorder: 1-3 years
- Experience in Sexual Abuse and Domestic Violence: 1-3 years
- Experience working with Culturally diverse populations: 1-3 years
- Hold/Maintain Ohio’s drivers license in good standing
Preferred education and experience
- CCDC or CCDC II certification

St. Vincent de Paul Society — Dayton District Council is a faith-based nonprofit that provides person-to-person assistance across the Miami Valley. Their programs include emergency shelter for women and families, supportive and permanent housing, food and clothing assistance, veteran supportive services, and a community thrift store (CityThrift) whose proceeds support their mission to help neighbors achieve stability and self-sufficiency.
