Job Summary:
- Coordinate with physicians, nurses, and other health team members to expedite medically appropriate, cost-effective care.
- Support physician provision of patient care with accountability for designated patient case load.
- Maintain and ensure adherence to clinical guidelines, tools, and protocols based on evidence quality-based medicine related to quality measure indicators.
- Promote effective utilization and monitoring of healthcare resources and communicates with the multidisciplinary team to achieve optimal clinical and resource outcomes.
- Apply process improvement methodologies in evaluating outcomes of care.
- Coordinate discharge planning needs to facilitate provision of services across continuum of care based on medical needs and financial feasibility.
- Utilize knowledge of the healthcare system to assist the patient in gaining access to services essential to continuity of care.
- Demonstrate and encourage team behavior and exceptional patient/guest experiences.
- Utilize nursing experience to assess patient's safe discharge needs and assist with appropriate transition to the next level of care per patient needs.
- Uphold and promote patient safety and quality.
Education:
Associate's degree in nursing or higher degree is required. Currently licensed by the state of Alabama. BSN preferred.
Experience:
Five years' clinical experience in acute care setting i.e., medical surgery critical care, ER, OR required.
Previous Home Health, Hospice, and/or Case Management Discharge Planning preferred.
Additional Skills/Abilities:
Skill and proficiency in applying highly technical principles, concepts and techniques that are central to case management. Advanced communication and interpersonal skills with all levels of internal and external customers. Must be proficient in Microsoft Word and Excel. Excellent organizational skills are required. Must be able to set priorities appropriately and handle multiple issues concurrently.
Skills Required
- Associate's degree in nursing or higher
- Current nursing license in Alabama
- Five years of clinical experience in an acute care setting
- Bachelor of Science in Nursing
- Previous home health, hospice, or case management discharge planning experience
- Proficiency in Microsoft Word and Microsoft Excel

- Excellent organizational skills and ability to prioritize multiple issues
