GA Foods is seeking a dedicated Community Support Coordinator to lead our community engagement efforts in California. This key position will be responsible for our outreach initiatives, driving member engagement with the programs, and managing relationships with referring entities. The CSC will play a crucial role in educating Medi-Cal beneficiaries about the meals and nutrition services they deserve, while serving as the primary liaison between GA Foods and various community health organizations.
Accountabilities:
- Develop and implement strategies to build awareness about the Community Supports (CS) program and GA Foods' Medically Tailored Meals (MTM) offerings throughout California communities
- Create and maintain a network of contacts within target populations
- Participate in CA Department of Health Care Services (DHCS) county events, health plan webinars, and seminars
- Represent the organization at CalAIM Collaboratives across all counties
- Develop and foster relationships with external partners to support CalAIM growth, including Community Based Organizations; Enhanced Care Management (ECM) and CS Providers; Clinics; Hospitals; Health systems; Federally Qualified Health Centers (FQHCs); and Community Senior Centers
- Plan, organize, and conduct outreach, visits, and in-service presentations to local referring entities to increase referrals
- Analyze and report on referral activities and performance, making recommendations for improvement
- Coordinate with cross-functional teams to ensure program deliverables and reporting meet requirements, exercising discretion in project management
- Stay informed about Community Supports integration with closed-loop referral systems and GA Food's presence
- Keep informed of DHCS and health plans' Community Supports reporting requirements
Requirements:
- 2+ years of community health work experience
- Associate or bachelor's degree is preferred, or relevant experience as approved by Human Resources
- Experience working with Medi-Cal
- Experience working with food and/or healthcare organizations
- Proven ability to work with cross-functional teams
- Excellent oral and written communication skills
- Strong presentation skills
- Proficiency in Microsoft Office suite (Outlook, Teams, Word, Excel, PowerPoint, and Power BI)
- High energy and desire to serve the community
- Organized with strong project management skills
- Valid driver's license, good driving record, and insurability
Preferred Skills and Experience:
- Successfully engaged with community health organizations
- Experience facilitating communication and client empowerment during interactions with healthcare and social service systems
- Experience in submitting health plans' reporting requirements by DHCS
- Experience participating in health plans' operational monthly/quarterly meetings
Additional Information:
- This is a remote position requiring willingness to travel regionally to events 50% of the time within California
- The role requires flexibility in scheduling and the ability to work independently to meet organizational goals

Frequent sitting, standing, and walking are daily activities. Some bending and filing may be required ( 10% of the time).
