Job Summary:

The Administrative Assistant/Targeted Case Manager provides comprehensive administrative support while coordinating case management services for individuals with developmental disabilities and families. This role is responsible for maintaining accurate records, managing correspondence, and ensuring efficient program operations. In addition, the position assesses individual needs, develops individualized service plans, coordinates referrals to community resources, monitors progress, and advocates for access to appropriate services. The Administrative Assistant/Targeted Case Manager works collaboratively with persons supported, healthcare providers, community support providers, and direct support professionals to promote positive outcomes while maintaining compliance with organizational policies, confidentiality standards, and applicable state and federal regulations. Strong organizational, communication, and problem-solving skills are essential to effectively balance administrative responsibilities with person-centered case management services.

Responsibilities:

  • Clerical Support:
    • Facilitate completion of AARP taxes. 
    • Establishes, maintains and revises office filing and other record-keeping systems for accurate retrieval.
  • Case Managers Duties and back up for Case Managers
    • Arrange for and secure supports outlined in the persons person centered support plan.
    • Research/Develop/Refer/Access community resources and services.
    • Seek modification of service systems when necessary to increase the accessibility and improve effectiveness of supports/services.
    • Communicate with the persons school district personnel, and local/state offices regarding the services and/or benefits of the persons served

Qualifications:

  • Bachelors degree or additional full-time experience in the field of developmental disabilities services, which may be substituted for the degree at the rate of six months of full-time experience for each missing semester of college.
  • One year of experience in administrative assistance preferred.  For a Bachelors Degree, must have six months of full-time experience in the field of human services.
  • For a non-Degreed, must meet all the requirements under Education plus a minimum of two years related experience.  One plus years of cases management experience preferred.
  • Knowledge of office equipment & personal computers
  • Utilization of time management skills.
  • Maintain confidentiality of individual served.
  • Ability to multi-task and meet deadlines.
  • Record, file, and transmit information.

Other Job Details:

  • Monday-Friday
  • 7am-3pm
  • Starting pay: $ 21.00
