Description
Greater Nashua Mental Health is seeking a compassionate and motivated ACT Case Manager to join our Assertive Community Treatment (ACT) team. In this role, you’ll work directly with individuals experiencing severe and persistent mental illness, helping them access services, build independence, and achieve recovery goals in their own communities.

What You’ll Do- Provide case management and direct support services to individuals with complex mental health needs
- Work within a multidisciplinary ACT team to coordinate treatment plans and services
- Support clients with medication monitoring, crisis intervention, and daily living skills
- Advocate for clients and connect them with community resources
- Deliver services in community and natural environments to support recovery and independence
- Assist with daily medication runs on a rotating schedule
- Participate in a rotating on-call schedule 1x per week
- Bachelor’s degree in Social Work, Psychology, Human Services, or related field
- 1–2 years of related experience preferred
- Strong communication, collaboration, and problem-solving skills
- Ability to work in a fast-paced, team-based environment
