JOB DESCRIPTION & REQUIREMENTS: This position will be responsible for facilitating review of ACO data to identifying and address opportunities for improvement, working together with the ACO Specialist. This person will build relationships with clinic staff, clinic managers and providers and serve as a resource for data pertaining to the ACO. This person will analyze managed care Pay for Performance data and work to attain related incentives. This is a collaborative effort to ensure we are meeting ACO goals, closing care gaps and attaining quality initiatives goals set by GAHHS and our ACO group, Illinois Rural Community Care Organization (IRCCO).
QualificationsRequirements:
Current RN or LPN licensure and 5 years of nursing experience required.
Prior ACO and/or clinic experience required.
Must possess excellent communication and time management skills.
Must possess computer skills and working knowledge of Office Word and Excel.
