Manage a Homeless Outreach program providing clinical and supportive services to adults experiencing homelessness and serious mental illness. Supervise, train, and evaluate staff; oversee admissions, discharges, documentation, client care, crisis response, medication and health concerns, billing, budgets, and grant management. Responsibilities include supervising and training multidisciplinary staff, coordinating client admissions and discharges, ensuring clinical quality and regulatory compliance, managing crisis response, monitoring client health and medications, collaborating with community agencies, and overseeing program budgets, billing, documentation, and adherence to the ACT model.

Assesses client needs, develops individualized service plans, monitors progress, provides crisis intervention, and connects clients with medical, vocational, housing, legal, entitlement, and family resources. Conducts intake and orientation, accompanies clients to appointments, supports community transitions, and maintains accurate documentation.

Provides community outreach, peer recovery support, client screening, case management, crisis intervention, treatment-plan coordination, health education, referrals, advocacy, and family support for ACT program participants. The role requires collaboration with clinical and community agencies, client assessments, rehabilitation services, and on-call coverage for nighttime and weekend crises.

Leads organizational quality improvement, compliance, accreditation, risk management, performance measurement, and policy management. Oversees CQI programs, audits, incident reviews, corrective action plans, quality dashboards, client satisfaction surveys, and accreditation readiness. Serves as Corporate Compliance Officer and Privacy and Security Officer, ensuring HIPAA and regulatory compliance. Partners with clinical and executive leadership to improve documentation quality, behavioral healthcare outcomes, and operational effectiveness.

Responsibilities include facilitating individual and group activities, case management, treatment planning, counseling, crisis intervention, family education, resource coordination, clinical documentation, benefits assistance, and collaboration with treatment teams and community providers. The role follows the Assertive Community Treatment model and may include nighttime and weekend on-call coverage.
