Provides comprehensive primary care to elderly PACE participants, including assessments, care planning, acute and chronic condition management, diagnostics, treatment coordination, discharge planning, and family education. Collaborates with an interdisciplinary care team to support seniors’ physical, emotional, and social well-being.

Assists the Director of Nursing with daily clinical operations, nursing staff supervision, performance evaluations, scheduling, audits, regulatory compliance, and quality resident care. Provides coaching and mentorship, collaborates with interdisciplinary teams, supports staffing and administrative functions, and serves as the Director of Nursing’s designee when needed.

Engages residents, supports resident activities, responds to prospective resident leads, conducts community tours, explains leasing options, follows up with prospects, and assists with marketing events and outreach. Maintains a welcoming community experience, supports social media content collection, organizes marketing materials, and builds positive relationships with residents, families, visitors, and prospective residents.

Maintains clean and sanitary resident areas, completes laundry, serves meals and refreshments, supports resident activities, follows safety and infection-control procedures, and builds positive relationships with residents, families, visitors, and team members.

Servers and dishwashers support dining services for long-term care residents. Servers set up dining areas, serve meals, assist residents, clear tables, and report concerns. Dishwashers clean dishes and kitchen equipment, maintain sanitation, handle garbage and floors, and assist with basic kitchen tasks. Both roles require teamwork, cleanliness, compassion, and adherence to health and safety procedures.

Coordinates interdisciplinary resident assessments, care planning, MDS completion and audits in a long-term care setting. Conducts resident interviews, calculates RAP triggers, monitors status changes, supports admissions and discharges, analyzes quality assurance data, reports findings to leadership, implements improvement initiatives, and trains staff on assessment and care planning procedures.
