Community Partners in Action Benefits Overview

Conduct resident intake, assessments, and individualized service planning in a supervised transitional housing program. Coordinate referrals and services, monitor resident behavior, administer PREA assessments and urine tests, maintain case documentation and data systems, prepare progress and discharge reports, and collaborate with courts, behavioral health providers, referral sources, and service teams. Support resident motivation, accountability, pro-social behavior, and respectful, inclusive program practices.

Provide gender-responsive case management for women involved in pre-trial and probation systems. Conduct assessments (LSI-R, ASUS), develop Individual Service Plans, coordinate community resources and referrals, facilitate CBT-informed groups, use motivational interviewing, maintain documentation and data in CDCS, deliver reports, and perform outreach to support rehabilitation and reduce recidivism.

Provide front-desk and administrative support for the Alternatives in the Community program: answer phones, manage files and client logs, conduct client intakes, maintain client records and CDCS entries, operate office technology, assist with interns, and coordinate clients in probation-related programs.
