Community Partners in Action Benefits Overview

Provide case management for residents of a supervised transitional house: conduct intakes and LSI-R assessments, develop individual service plans, coordinate community referrals, monitor behavior, complete documentation and data entry (CDCS, Securmanage), supervise urine tests, perform PREA assessments, participate in team meetings and trainings, and collaborate with referral sources to support reentry and reduce recidivism.
